Two miners working in an underground bin excavation were attempting to level the muckpile by blowing muck down a hole at the brow. They were both tied to retractable fall restraints when the muckpile slid down the repose angle unexpectedly. One employee manually pulled his line and engaged his restraint but the second man was engulfed and suffocated by the finely broken muck.

A development miner was installing 12-foot rebar bolts in the back with a jackleg drill. The previous bolt that they attempted to install set up before it was driven so there was about 8" of the bolt exposed. The miner was holding on to the bolt and their hand slipped off the bolt causing it to spring back & strike the employee in the upper lip resulting in a laceration.

Hoistman was signaled to move north conveyance into position for weekly cage inspections at the #2 shaft top station and the hoist tripped out on YA1 Converter fault. Electricians and an ABB technician performed troubleshooting and identified the cause to be a faulty breaker.

A mobile maintenance mechanic was changing out a grease barrel on a lube truck and when they were picking up the pump and lid to reinstall it on the drum, they lost grip of the pump and when they attempted to catch it, the sharp edge of the lid on the pump caused a laceration on their left thumb requiring two sutures.

A jumbo carriage was being lowered using the Service Hoist & was suspended under the conveyance & shoed to the shaft guides. At the 3,500' level, the hoist program detected a change in weight resulting in the hoist program stopping the hoist. Shaft repair crew went to inspect and at 10:47 AM, it was determined that the shoes came off the guide making the hoist unavailable for use.

At 4:15am the hoistmen was attempting to clutch in when the weight brake set and EE could get it to release. Maintenance was called and found that a safety was tripped causing the hoist to dump air pressure setting the weight brake. maintenance reset the safety bringing the hoist back into operation.

Silver Shaft hoist operator heard noise coming from the drum while the hoist was in operation. The hoist operator called maintenance to come inspect the hoist and they found that a saddle and both nuts on a Crosby clamp had come loose. The clamp that came loose was one of 4 extra clamps that were added to hold the tail of the rope on the inside of the drum.

The hoist tripped due to the gap on the right brake stand-2 upper caliper was detected on the hoist control system to be outside of tolerance at 2mm. The gap was checked and adjusted to 0mm at which time the hoist was reset and back operational. The Silver Shaft Hoist (primary) and 2 Shaft hoist (secondary) were operational throughout.

Lost incoming power to the mine site for a short period of time due to a weather event which caused the hoists to trip. The hoists and compressors needed to be reset and started back up. Hoists were back operational by 7:04 PM.

Employee was transporting a tote of diesel fuel and got a flat tire on the 1-yard forklift they was using to transport the fuel. Employee was pulling into a cutout to get it out of the roadway and as they were pulling in they hit a bump and reached their arm out toward the rib and strained left shoulder. Employee ended up having to get surgery for a rotator cuff injury.

Miner was mucking into a muck bay with a 2-Yard LHD. They had a full bucket and when attempting to dump it they got on uneven ground and as they raised the bucket the loader tipped over onto the right side. After it tipped, oil and fuel began to leak onto hot components of the exhaust and possibly the turbo which caused the fluids to ignite, resulting in the loader catching fire.

At 2:20 the hoistmen tried to start 2 shaft hoist but it would not start. Electricians were called to diagnose problem and found the pinion break pressure was at 70 psi which needed to be 1500 psi to start the hoist. It was determine that the oil filter was partially plugged causing the issues.

It was determined that the YA1 converter breaker was defective and needed to be replaced. After consulting with ABB's drive specialists and changing out the breaker, the service hoist was tested multiple times without any erroneous YA1 faults and was deemed back in good operation.

At 10:30 pm there was a report that 2-shaft top station was hazier than normal but gas readings with a Ventis pro 5 gas detector were normal. A supervisor bump tested an MX6 gas monitor & accompanied employees back to 2-shaft portal to investigate. When the 2nd air door was opened, there was smoke so they retreated & initiated the mine emergency response plan.

EE pushed the 4' split set bolt into the drilled hole in the back by hand. As EE was positioning self to drive the bolt the rest of the way in with the jackleg, the bolt fell out of the hole and struck EE on the right forearm.

EE was removing an electrical jumper cable from the rib and felt a "pop" in EE's stomach. EE was diagnosed with a small hernia. EE continued mining at full capacity. EE elected to have a procedure to fix the small hernia on 7/3/2023. EE returned to work after procedure on EE's next scheduled shift of 7/8/2023.

A breaker for the battery backup for the #2 hoist tripped during a severe weather event during the night. This caused the batteries to wear down because they could not charge due to the tripped breaker. Once the battery level got low enough, it tripped another breaker to the Grinnell System. This caused the hoist to shut down.

EE was struck in the face by a rock that was dislodged from the face. The miner was installing a bolt into the fingers of the bolter when the bolt hit the face, a rock slide down the bolt and struck the employee.

After the Silver Shaft Hoist was idle for a lengthy time the weight brake would not lift fast enough or high enough to release. A consultant was called in to help diagnose the problem and identified that the brake set regulator pressure was not high enough to engage the brake boost solenoid. The regulator pressure was turned up correcting the problem.

All personnel were hoisted from the mine at 08:10 am and MSHA was notified (Boise Office). At the time of the incident there were 48 Lucky Friday employees and 31 Contractors UG.

The hoist was restarted, tested, and returned to full service at 9:58 am.

The electrician's removed and replaced the hoist controller thus fixing the problem. The hoist became operable at 7:55 AM, was tested, and returned to duty at 8:10 AM.

A rockburst hit the 08 stope at 8:32 am with no damage occurring. a small series of rockbursts continued for the next 45 minutes. around 9:20 am the decision was made to withdraw the miners from the stope for precautionary reasons.
