On 1.31.26 between 3:00 PM and 4:00 PM Staderian, employee fell twice while attempting to de-ice a conveyor that had began to travel differently due to weather conditions at the time. ( Miner is currently out pending Orthopedic evaluation)

On 12/1/2025, (reported 12/3/2025) Miner was removing belt clips over K1stone tunnel belt with a grinder. Small piece of debris went under eye protection hitting EE's right eye. Attempts to flush eye were unsuccessful, employee received additional treatment at local eye doctor.

Employee was moving railcars at the loadout area of the plant. Employee attempted to adjust/tighten hand brake on a railcar. In the process of doing so the employee's left quadricep was pinched between the couplings on 2 railcars. Employee was taken to local ER for treatment and evaluation. At this time the employee is still receiving treatment at the facility.

The employee was in the process of loading railcars at our loadout area. While opening and closing lids employee strained their back. On 6/12/2022 employee notified supervisor of pain again. Employee was taken to a local medical facility for evaluation. Where it was determined a strain and was later given physical therapy prescription.

While helping with a repair at the hydrator the Miner slipped at the loading dock area. The slip resulted in an injury to the right knee. The Miner is on restricted duty. They were removing material from the elevator and the ground was wet from rainy conditions.

The miner was cleaning metal from the stone belt magnet by hand. EE was wearing leather driver gloves. EE attempted to pulling a 5 inch X 3 inch X 1.5 inch metal fragment from the belt magnet and cut a finger when their grip slipped. The miner did not see the sharp edge shavings attached to the metal and pulled across them cutting the right index finger requiring stitches.

The Employee Was Cutting Off A Metal Section Of The # 2 Feed Tunnel. When The Metal Fell Off And Hit The Torch Line Pull The Torch From His Hand On To His Left Forearm. The Metal Tip Hit His Glove Pulling It Down And Contacting His Skin. The Contact Caused 2nd Degree Burns To The Left Forearm.
While stepping on to the Cat 325c excavator SR # Bfe1984 located on the North Quarry lower level Ramp, The Miner Slipped on the step and reached to steady his self. The miner's wedding band caught on the lower wiper blade cradle bending the band and cutting the bottom of the ring finger on the left hand. The miner received four stitches on the bottom of the ring finger.

While emptying a trash Barrel the employee pulled on the lip at the bottom of the barrel with his right hand to flip it over into a loader bucket. During the lift he pulled the middle finger causing it to hurt and swell 2 weeks later. Miner went to the doctor on 10/17/2012 and did not miss any time. He did receive a prescription making this a reportable injury.

While climbing a ladder in the hydrator area a mass of lime fell striking EE. The mass was from the wet lime on a boot that had stuck to a rung above him. The safety glasses were dislodged and the mass landed in the right eye. The contact caused chemical burns and no vision impairment. He received a eye cream. The miner had no restricted duty/lost work days.
