On 12/1/2025, (reported 12/3/2025) Miner was removing belt clips over K1stone tunnel belt with a grinder. Small piece of debris went under eye protection hitting EE's right eye. Attempts to flush eye were unsuccessful, employee received additional treatment at local eye doctor.

Contractor (SF9) Drill operator was attempting to drive equipment to next drilling location. Investigation is on-going

Employee was moving railcars at the loadout area of the plant. Employee attempted to adjust/tighten hand brake on a railcar. In the process of doing so the employee's left quadricep was pinched between the couplings on 2 railcars. Employee was taken to local ER for treatment and evaluation. At this time the employee is still receiving treatment at the facility.

Loading a shot, stepped on uneven ground, pain in right leg/hip (1st day of treatment 7/21/2022).

The employee was in the process of loading railcars at our loadout area. While opening and closing lids employee strained their back. On 6/12/2022 employee notified supervisor of pain again. Employee was taken to a local medical facility for evaluation. Where it was determined a strain and was later given physical therapy prescription.

Employee was shoveling material and strained a muscle in their lower back. Employee was treated and returned to work with no restrictions.

Employee was walking off burner floor and fell while descending a two-stair area within the plant, landing on their side. Injury was not reported until 07/07/2021 and employee was treated for shoulder strain.

Employee was assisting with removing a pulley from a conveyor when they attempted to lift one side from an awkward position, causing a hernia.

While helping with a repair at the hydrator the Miner slipped at the loading dock area. The slip resulted in an injury to the right knee. The Miner is on restricted duty. They were removing material from the elevator and the ground was wet from rainy conditions.

The miner was cleaning metal from the stone belt magnet by hand. EE was wearing leather driver gloves. EE attempted to pulling a 5 inch X 3 inch X 1.5 inch metal fragment from the belt magnet and cut a finger when their grip slipped. The miner did not see the sharp edge shavings attached to the metal and pulled across them cutting the right index finger requiring stitches.

While inspecting the 4 & 5 truck load out chute repair employee's right ring finger tip was pinched off into their glove. Employee was checking the slack in the cable when a second employee started to raise the chute pinching the finger between the cable & pulley.

While raking the grates on the #1 kiln The miner pinched left small finger between the metal hoe handle and the building support beam. EE was wearing drivers gloves. A X-ray was taken showing a fracture to the left hand small finger distal phalanges.

EE Was walking down the ramp to the #2 cooler pit to repair a water pump. He slipped fall back onto another miner bumping his right elbow on the concrete floor.

Employee Was Walking Down Into The # 2 Coolier Pit When His Right Leg Slipped And He Fell Folding His Left Leg Under Him. The Knee folded under him on the sloped concrete floor.

Driver was unloading at the incident location when the hose fitting separated from the silo and blew hydrate lime in the drivers eyes.

Employee Cut His Left Arm Between The Wrist And Elbow. The Injury Occurred While He Was Cutting A 1 Inch Water Hose With A Razor Knife At The Rear Of The # 2 Lime Kiln Stairway. He received 3 stitches and returned to work.

Miner was raking grates over the course of his weekly shift which caused right wrist pain. Miner has an existing injury to the same wrist non work related. The miner received a prescription for swelling.

While racking grates he strained the left shoulder.

Employee was raking the kiln grates. He was pulling a large piece off the grates when he felt a pull in his left shoulder.

While bending over to lift the plastic vac hose EE experienced pain in his upper back below the left shoulder. EE had not lifted the pipe and was located at the vac pipe storage area near the boom truck shed.

The Employee Was Cutting Off A Metal Section Of The # 2 Feed Tunnel. When The Metal Fell Off And Hit The Torch Line Pull The Torch From His Hand On To His Left Forearm. The Metal Tip Hit His Glove Pulling It Down And Contacting His Skin. The Contact Caused 2nd Degree Burns To The Left Forearm.

While stepping on to the Cat 325c excavator SR # Bfe1984 located on the North Quarry lower level Ramp, The Miner Slipped on the step and reached to steady his self. The miner's wedding band caught on the lower wiper blade cradle bending the band and cutting the bottom of the ring finger on the left hand. The miner received four stitches on the bottom of the ring finger.

While Blowing Out A Transfer Chute With An Air Lance At The Rear Of The # 2 Kiln Limestone Particles Flew Into EE's Left Eye Behind His Safety Glasses.

While emptying a trash Barrel the employee pulled on the lip at the bottom of the barrel with his right hand to flip it over into a loader bucket. During the lift he pulled the middle finger causing it to hurt and swell 2 weeks later. Miner went to the doctor on 10/17/2012 and did not miss any time. He did receive a prescription making this a reportable injury.

While climbing a ladder in the hydrator area a mass of lime fell striking EE. The mass was from the wet lime on a boot that had stuck to a rung above him. The safety glasses were dislodged and the mass landed in the right eye. The contact caused chemical burns and no vision impairment. He received a eye cream. The miner had no restricted duty/lost work days.

Employee was climbing down a ladder. His foot slipped and twisted lower right side of back.

Employee was walking to the shop and stepped on a rock. He heard something pop in his foot. He broke a bone in his foot

Employee was cleaning lime pit. He received lime burn on both legs. He notified employer on 5/31/2011. Burns became infected and went to the doctor on 6/4/2011.

Employee slipped and fell off a ladder. He was about three feet off the ground. He bruised a rib on his rt. side.

The employee was not working at this time. His body was found on mine property. It appears to be suicide.

He was drilling a piece of metal with a hand held drill. The bit hung and the drill twisted his right wrist.

He was waling with a steak knife in his hand, he tripped and fell on the knife, sticking the knife in his right thigh.

He was installing O2 probe. The probe slipped and mashed his finger. The finger was just bruised.

He was removing trunion bearing housing. When the housing came free it swung and pulled his right shoulder. He sprained his shoulder.

Employee was working on th #2 kiln dust elevator and dust from the elevator got into his eyes.

Injured right hand while attempting to work on a dewatering unit squeezing a pair of pliers.

He was using a sledge hammer to drive a pin. A piece of metal broke off the head of the sledge hammer and lodged in his right thigh.

Employee was bagging hydrate lime. A bag busted and the hydrated lime went into his eyes. The lime irratated his eyes.

EMPLOYEE WAS CLIMBING DOWN A LADDER AND WHEN HE STEPPED TO THE GROUND HE SLIPPED AND SPRAINED HIS LEFT ANKLE.

Employee was using chain hoist. He was pulling on the ratchet and pulled a muscle in his lower back.

Employee was woring on screw conveyor. He slipped and tried to catch himself. He landed on his right shoulder. He starined his shoulder.

Employeee was operating bagging machine. A bag busted at glue seam and hydrated lime got into his eyes.

He was removing a hydraulic cylinder from a preheater ram, when the cylinder was removed, the ram moved pinching his right leg.

He was performing normal duties at preheater, he became over heated.

Employee was removing coal mill roller from coal mill. When the roller came free his right hand was caught between the roller and the burner pipe. He suffered a cut to his right hand.

Employee was lifting a cooler grate with the help of another employee. He strained his lower back.

Employee was operating palletizer at hydrate building, he reached to remove an empty bag and his right hand was caught between a roller. His right hand was cut between thumb and forefinger.

Employee was exiting a 936F wheel loader, while climbing down the steps he slipped and twisted his left ankle.

The employee fell from the bucket of a skid steer loader, he bruised his right hip and his right leg and his right knee.

Employee was positioning forks on a forklift and the fork slipped pinching his right ring finger, cutting and fracturing his right ring finger.

Employee was standing on rooler conveyor and fell down. He fractured his left thumb and sprained his right ankle

Employee was using a pry bar installing a screw conveyor. The pry bar slipped and the screw conveyor hit his right hand. He sprained his right wrist.

Employee was leaving his work station for the day. He slipped and fell on the ground bruising his lower back.

Employee had been welding in the shop. He was walking and stepped on a piece of steel. He sprained his right ankle.

Employee was exiting a wheel loader and when he stepped onto the ground, he twisted his left ankle. Resulting in a sprained left ankle.

He was working on semi-automatic palletizer and got his right hand caught in roller. He had a compression injury to right thumb and contusion to top of right hand.

He was welding on an I.D. fan. He started feeling bad. he was overcome by fumes. He was treated at hospital and released six hour later.

EE WAS INSIDE KILN REMOVING AN ASH RING MATERIAL FELL FROM OVERHEAD AND STRUCK HIM ON HIS BACK AND RIGHT LEG. HE HAS ABRASIONS AND BRUISES ON HIS BACK AND RT LEG.

EE WAS USING A RUBBER MALLET TO REMOVE MATERIALFFROM A SILO. WHILE SWINGING THE MALLET HE HURT HIS LEFT SHOULDER.

EE WAS HELPING TO LOAD TRAIN CARS HE WAS.

ANOTHER EE WAS CUTTING A PIECE OF METAL AND HE WAS STANDING AND WATCHING. A PIECE OF METAL FLEW OFF AND HIT HIS HAND AND RIGHT LEG. CUTTING HANDAND BRUISING LEG.

HE WAS STANDING ON A 16" CONCRETE BLOCK. HE FELL ON HIS RT ARM. BROKE BONE IN RT WRIST.

HE WAS WORKING ON QUICKLIME ELEVATOR, AND EXPOSED TO QUICKLIME. 2ND DEGREE BURNS ON RT. ARM & ARMPIT.

PULLING ON BAGHOUSE DOOR, PULLED MUSCLE IN BACK.

EMPLOYEE WAS USING SHOVEL TO CLEAR UP A ROCK SPILL. HE GOT TOO HOT AND PASSED OUT.

HE WAS USING A HAMMER TRYING TO REMOVE A CAOL MILL ROLLER. A PIECE OF METAL BROKE OFF AND LODGED IN HIS RT HAND.

LIFTING LOADED PALLET OF HYDRATED BAGS, RIGHT LINGUINAL HERNIA.

INJURED WORKER WAS ON A TANKER ATTEMPTING TO OPEN A HATCH. HE SLIPPED AND FELL FROM THE TANKER, LANDING ON THE GROUND.

EE WAS WALKING AND STEPPED ON A LUMP OF COAL. HE TWISTED HIS RT. ANKLE. SPRAINED RT. ANKLE.

EE WAS HAULING STRIPPINGS. LARGE BOULDERS LOADING ON TRUCK SHIFTED TRUCK AND HURT HIS LOWER BACK.

HE WAS EXAMINING TOE OF STRIPPING FACE. A ROCK FELL AND ROLLED ON HIS LEG AND FRACTURING RIGHT ANKLE.

EE WAS LIFTING 5 GALLON BUCKET OF OIL FROM THE GROUND TO A 4' PLATFORM. WHILE HE WAS LIFTING HE HURT HIS LOWER BACK (MUSCULAR LOWER BACK PAIN).

EE WAS CLIMBING DOWN LADDER AND THE LADDER SHIFTED HE JUMPED ABOUT 3 FT TO THE GROUND AND TWISTED HIS ANKLE. HE SPRAINED HIS RT. ANKLE.

EE WAS TRYING TO FREE A CONVEYOR ROLLER, BY TURNING IT WITH HIS HAND HIS GLOVE WAS CAUGHT IN THE CONVEYOR BELT AND PULLED HIS HAND BETWEEN THE ROLLER AND THE BELT.

HE WAS USING A CHIPPING HAMMER AND A PIECE OF METAL BROKE OFF AND PUNCTURED HIS LEFT HAND.

ONE PIECE OF SIX INCH STEEL PIPE, STRUCK THE EE IN THE CHEST AND FRACTURED HIS CLAVICLE.

EMP WAS DRIVING SERVICE TRUCK ON A HAUL ROAD AT CHENEY LIME QUARRY. DROVE OVER A ROCK CAUSING THE STEERING WHEEL TO SUDDENLY SPIN. ATTEMPTING TOSTOP THE SPINNING STEERING WHEEL, HIS RIGHT HAND WAS HIT SEVERAL TIMES.

QUICKLIME FELL INTO RIGHT EYE. CHEMICAL BURN OF RIGHT EYE.

HYDRATED LIME FROM BALLMILL WAS BLOWN INTO EYES CAUSING A CHEMICAL BURN.

ROCK FELL OFF CONVEYOR BELT AND STRUCK EE IN HEAD CAUSING SLIGHT CONCUSSION.
