Employee was putting guards on a conveyor with a drill and drill bits. They slipped, and caught their left arm on some angle iron. They cut their left arm. They received a tetanus shot, they cleaned up the arm, bandaged it, and sent them back to work without restrictions.

Employee is an electrician, and was working on wiring. EE was using EE's knife, and poked EE's finger. It broke the skin but did not bleed. Then EE found a dead raccoon, that had been chewing on the wiring. After consultations, there was a concern for the possibility of rabies. Employee started a series of rabies shots as a precautionary measure.

Employee was using a razor knife to cut line wrap, and the blade broke. When EE was changing the blade, EE cut right hand. EE went to the doctor, and they stitched the wound.

Employee was getting out of a front-end loader. EE had one foot on the ground and the other on the loader step. The ankle on the ground rolled over. EE lost balance and fell backward, hitting EE's back on a 4x4 piece of wood. EE has thoracic compression fractures and a sacral fracture.

Employee was replacing parts on a screen deck. EE lost EE's balance, and fell about 7' and landed on a conveyor. EE fell on EE's left leg, and had some pain in EE's left arm, but went back to work. EE sought treatment a few days later. EE had an MRI, which came back today. There were no breaks or torn ligaments. PT was recommended. EE did not miss any work.

Employee was cleaning out the loader bucket with a scraper. EE jarred right shoulder, but did not seek any treatment until December 1. EE went to a clinic, and they suggested physical therapy, which helped. EE still had pain, so EE got an x-ray and MRI. It was then discovered EE a had a torn rotator cuff. EE is scheduled to see an ortho surgeon for further evaluation.

Employee was putting a new belt on a conveyor. Employee was using a grinder to remove the belt clip's bolts, when a small piece of debris got into their left eye. Employee was wearing safety goggles when this happened.

On November 2, employee was using sledge hammer to repair crusher. He said the last hit felt funny. The next day, his hand swelled up. On November 4, he went to doctor. Doctor said he had slight fracture to right hand. The doctor gave him splint, placed him on restricted duty. Chris went for follow-up on November 13. Doctor released him to full duty as of November 14.

Employee was removing scrap repair items from a loader bucket and throwing them into a dumpster. He got a little piece of something in his right eye. He used eye wash to wash out his eye. He said it felt fine, so he went back to work. That night, his eye was bothering him, so he went to an urgent care facility. They told him he had a corneal abrasion.

EE was doing EQ. maint. He was pulling out an anvil in the VSI when he felt something pull in his groin area. He went to the Dr. on 10/21. They referred him to a surgeon that appt. is 11/7. We will know more at that time as whether he needs rest or surgery, if he will be off work and how long he may be off.

In July, EE complained that he had a sore neck. The doctor who examined him said some of the problem was due to aging (general wear & tear) and some was due to the type of job; he wasn't sure. EE had an MRI. The result said he had cervical neck syndrome, with 4 or 5 different diagnosis. Last week, they finally decided that his injury was probably more work related than hereditary.

On December 1, the EE hit his thumb with a hammer while pounding out a bolt. The next day, December 2, he was removing a book from a book shelf and the book slipped, hitting his swollen thumb. He cut his thumb on the bottom of the book shelf.
