Aggregate Resources Gravel Pit Metal/Non-Metal

Employee was using a pressure washer to clean debris from underneath a truck scale. While doing this, the nozzle slipped, causing an abrasion on the back of the employee's right hand, similar to a burn. Employee sought treatment. The hand was wrapped and employee was given an antibiotic.

Sometime in January (employee is unsure of date), employee was lifting a steel beam. EE picked it up and put it on EE's shoulder. EE felt pain in EE's left shoulder. The pain persisted for several weeks, but EE did not report the injury until April. EE had EE's shoulder checked, then had an MRI. It appears EE will need surgery.

Employee was using a sledge hammer to pound a nut on the mantle of a coarse crusher. EE felt something in the right elbow. It resulted in swelling and loss in range of motion.

Employee was walking, slipped on ice. Theye twisted right knee. EE didn't want to see doctor. It still hurt on Feb. 13 so they visited doctor. They told them to ice it, wear compression sleeve. EE went for follow up on Feb. 20. It still bothered them, so was sent for x-rays. EE has another followup on Feb. 27. Should have results and more information at that time.

Employee was working on an auger; EE slipped and hit EE's right knee on the auger's edge. EE finished the work day and went home. Later EE's knee swelled up, was bruised. EE went to the doctor the next morning. EE got an X-ray, which didn't show anything. EE returned to work, without any restrictions, limitations, prescriptions, or further treatment.

Employee was using an air compressor with an air sprayer to clean out the cab of truck. When employee was on way home that night, right eye started to bother. Went to the doctor the next day, and the doctor removed a foreign object from eye.

The EE was changing a screen panel on processing plant. He threw the old panel over the plant hand rail. The panel fell back. It hit the EE, knocking his hard hat off & hitting him on the right side of his head near his temple. The EE finished his shift, but felt dizzy. The next day, he still felt dizzy, so he went to the doctor. The doctor determined the EE suffered a concussion.

Employee was using an iron bar to hammer in the skirt rubber on a conveyor. He missed or slipped, and hit his right hand on the edge of the hopper box. He broke the pinky finger on his right hand.

EE was working in a van, roll-up door was open part way. Was climbing in. Part of body & one leg were in,& was pushing himself the rest of the way in. Foot slipped, & scraped his right shin on the edge. Continued working. That night, noticed a big bruise on his leg, and it was swollen. The next day, it was worse, so he went tot the ER. He had a strep infection in his skin.

EE was cleaning out scale. As he was putting the top doors back on, he felt a "pop" in his left elbow, followed by pain and cramping in his left bicep.

Employee was repairing a primary crusher, when he strained some muscles in his groin. He saw the company doctor, who diagnosed the injury as a hernia. He was referred to a surgeon. NOTE: The employee did not report the injury to the employer until Sept 19

EE was changing screens when he pulled some muscles in his stomach area. He waited a couple of days to see if it would improve. It didn't, so he went to see the doctor on July 26th. He was diagnosed with an umbilical hernia. It is not restricting him, so he decided to wait until this fall to have it reassessed and to schedule surgery.

Employee dropped a pipe wrench on his hand, 4 days later he told his foreman he had a hard spot on his wrist.

Employee was using a cutting torch to cut material. Apparently, the connection was loose where the torch and hose came together. This caused the fitting to leak a little. The torch ignited, burning the employee's left hand.

Employee was welding off and on all day on a jaw crusher, doing repairs. He was wearing a welding helmet, but he was ina tight spot, so it was difficult to get the mask down in time, every time, before he started welding. He had welding flash as a result.

Employee was working in an area where another employee was doing some welding and was accidently exposed to welders flash. After work, later that night, he had problems with his eyes, so he went to a doctor for treatment.

EE WAS REPAIRING A CRUSHER. HE STEPPED UP ON THE LEDGE OF THE CRUSHER TO TIGHTEN A BOLT, WHEN HIS FOOT SLIPPED AND HE FELL 3 FEET, BREAKING HIS WRIST.

EE WAS TWISTING A WIRE TIE, WHEN HIS HAND SLIPPED AND HE CUT HIS LEFT INDEX FINGER

EMPLOYEE WAS CLIMBING DOWN HE LADDER TO EXIT A FRONT END LOADER. HIS FOOT SLIPPED ON THE LADDER. HE LOST HIS FOOTING, HITTING HIS LEFT RIBS ON HIS LEFT SIDE ON THE LADDER, AND PARTIALLY TEARIN G HIS RIGHT BICEP MUSCLE WHILE TRYING TO HOLD ON.

EMPLOYEE WAS GREASING THE PINS ON A FRONT-END LOADER. WHEN HE WAS FINISHED, HE STEPPED BACK, BUT MISSED HIS STEP, FALLING DOWN. HE TRIED TO CATCH HIMSELF WITH HIS RIGHT ARM, BUT HE CAUGHT IT O N PART OF THE LOADER, AND PULLED HIS SHOULDER OUT OF JOINT.

EMPLOYEE CUT HIS FINGER ON A PULLEY, REQUIRED 8 STITCHES.

EE WAS USING A TORCH TO CUT METAL & HE BURNED HIS RIGHT HAND

EE WAS TIGHTENING A BOLT WITH A BOXED END WRENCH. THE WRENCH SLIPPED OFF THE HEAD OF THE BOLT CAUSING EE'S MOMENTUM TO MOVE HIM BACKWARDS RESULTING IN STRAIN TO RIGHT KNEE.

EMPLOYEE STATES THAT HIS KNEE HAD BEEN HURTING AND HAD SOME SWELLING FOR SEVERAL DAYS ON 9TH,HE STEPPED IN THE MUD AND TWISTED KNEE.THUS RE-AGGRAVATING OLD INJURY (11-09-92).

EMPLOYEE STATES THAT HE WAS STANDING IN DROP BOXPULLING ON POWER CABLE. HE TWISTED, PULLING MUSCLE IN SHOULDER.

EE STATES THAT HE WAS WORKING ON A RETURN ROLLER AND AS HE STOOD UP TO WALK AWAY A ROCK CAME OFF THE CONVEYOR BELT HITTING HIM ON THE HAND.

EE STATES THAT HE WAS WALKING UP A PORTABLE CONVEYOR, WHEN HE LOST HIS BALANCE AND FELL, LANDING ON THE SIDE OF THE CONVEYOR WITH HIS RT SIDE IN THE RIB AREA.

EMPLOYEE STATES THAT HE WAS DECENDING STAIRWAY STEPPED ON A STONE & TWISTED HIS KNEE

EMPLOYEE STATES THAT ON 11-3 HE WAS INVOLVED IN SHOVELING GRAVEL FOR APPROX 8 HRS AND THAT HE EXPERIENCED PAIN AND SWELLING IN HIS FOREARM THAT EVENING. ON 11-4 EMPLOYEE REPORTED TO FOREMAN. EMPLOYEE WORKED LIGHT DUTY JOBS THAT DAY ON 11-5 EMPLOYEE REQUESTED TO SEE DR. DIAGNOSIS WAS TENDENITIS OF LEFT FOREARM.

EMPLOYEE STATES THAT HE WAS DRAGGING A WATER HOSE IN A CROUCHED POSITION AND PROCEEDED TO STAND UP, HITTING HIS HEAD ON A OVERHEAD BEAM.

EMPLOYEE STATES THAT WHILE SHOVELING UNDER THE DROPBOX A STONE STRUCK HIM ON THE RT HAND CONTINUED TO WORK OUT SHIFT REPORTED TO WORK ON MON 05-03-HAND SWOLLEN REQUESTED TO SEE DOC

ELMPLOYEE STATES HE WAS UNPLUGGING CHUTE. WHEN PREPARING TO STEP DOWN OFF LEDGE HIS FOOT SLIPPED AND HE FELL APPROX 4 FT TO FLOOR.

EMPLOYEE STATED HE HAD PAIN IN LEG.HURT HIS BACJK SHOVELING,WENT TO DOCTOR IN AFTERNOON,GIVEN PRESCRIPTION,RETURNED TO WORK.FOLLOW UP VISIT TO DOCTOR;OFF WORK AS OF 05-29-92 PER DOCTOR.

SLIPPED ON SPRING OF SCREEN WHILE CHANGING SCREENS AND LANDED ON EAR OF SCREEN.

HE WAS WORKING OFF THE F2 CONVEYOR AND WAS PULLING ON THE BELT WHEN HE HURT HIS BACK.

WHILE REPLACING A LINER THAT WEIGHED APPROX 100LBS HE LIFTED & TURNED TO THE LEFT RAISING THE LINER OVER HIS HEAD REALIZING TOO LATE THAT IT WAS TOO HEAVY & DROPPED THE LINER THAT NIGHT HE FEL T PAIN IN HIS BACK & REALIZED THAT IT WAS PROBABLY DUE TO THE INCIDENT ABOVE

WHEN TURNING THE BOWL OUT OF THE STANDARD CRUSHER BY HAND, HE TURNED WRONG AND TWISTED HIS BACK.

WHILE MODIFYIN SCRUBBER BASE, HE REACED UNDER PLATFORM TO CUT A WELD THAT WAS MISSED. WHEN WELD BROKE, PLATFORM DROPPED APPROXIMATELY 6" BECAUSE OF INSUFFICIENT STRAIN ON CABLE OF CRANE HOLDI NG PLATFORM. WHEN PLATFORM DROPPED IT STRUCK HIM ON BILL OF HARD HAT AND KNOCKED HIM BACKWARDS FROM A SQUATTING POSITION. TORCH HE WAS USING BECAME ENTANGLED IN CLOTHING.

SWINGING SLEDGE HAMMER IN TIGHT QUARTERS AND IT KEPT HITTING HIM IN RIBS.

UNHOOKED A GAS WELDER FROM CAR.

PULLED LOW BACK WHILE LIFTING A BEARING OUT OF A BUCKET

LIFTING CHAIN HOIST AND PULLED MUSCLE IN UPPER ABDOMEN.

SPRAINED WRIST PUTTING UP STEEL PLATE ON F-7 CONVEYOR

WAS PULLING A CABLE OUT O F THE BUCKET OF THE BOBCAT WHEN HE FELT A PAIN IN HIS BACK WHICHW AS THE RESULT OF A PULLED SHOULDER MUSCLE IN HIS BACK.

RUPTURED DISK AS STATED BY THE PHYSICIAN EMPLOYEE ALLEGES THE OPERATION OF THE WHEELED LOADER AGGRAVATED HIS BACK.

STRAINED BACK LIFTING STEEL PLATE 16"X24"X3/8" INSIDE DROP BOX

WAS REPLJACING 3 LB CA( IN TROUGHING IDLER WHEN E FELT SHARP PAIN IN BACK IT WAS COLD AND STA\T OF SHIFT HE MUSCLES WERE STIFF.

EMPLOYEE FELT IT NECESSARY TO CLIMB ONTO A BOARD ON TOP OF THE CONVEYOR WHICH WAS SITTING AT A SLIGHT ANGLE. THE BOARD ROLLED DOWNHILL CAUSING HIM TO FALL. HE HIT HIS LEFT KNEE ON THE ROCKS CUASING SORENESS AND SWELLING WITH TWO SMALL CUTSA ON KNEE CAP.

EMPL REPORTED TO WORK IN BEGINNING OF WEEK WITH COMPLAINT OF SORE BACK PLAYING SOFTBALL & LATER IN WEEK EMPL ALLEGESS THAT ADDITIONAL INJURY WA CAUSED WHILE HE WASS REPARING SCREENS & CHUTE LI NERS IN GRAVEL PLANT.

EMPLOYEE WAS CUTTING AND WELDING IN SCRUBBERS WIND BLEW RUST FROM METAL IN SCRUBBER INTO HIS RIGHT EYE.

SUBJECT EMPLOYE9 HAD TAKEN SAMPLE OF CURRENT PRODUCTION MATERIAL AND WAS WALKING DOWN OF STOCKPILE LOST HIS FOOTING AND HIS BODY REACTION CAUSED HIS KNEE TO TWIST. REASON FOR DELAY IN SUBMISSI OF THIS REPORT3IS THAT EMPLOYEE SEEMED TO BE ALRIGHT. DURING THE REMAINING OPERATING SEASON THE KNEE BECAME AGGREVATED MORE AND MORE. DR RECOMMENDED SURGERY MISCROSOPIC AT THE TIME 1-13-87.
