Aggregate Resources, Inc. · Caught in, under or between a moving and a stationary object

A rock was caught in tail pulley. Employee turned conveyor off, locked it out, and loosened belt to remove rock. Employee tried to turn the belt to roll the rock out. When the rock came out, the belt shifted, and the pulley grabbed employee's glove, pinching left index finger, causing a fracture.

EE was adjusting jaw crusher. He put a pipe over the wrench, for leverage, & pushed on pipe. When he did, he said something in rt shoulder snapped. He said he told his supv about it, but didn't seek treatment thru employer. Employer found out several wks later that EE was being treated by his personal dr. Inj was determined to be work-related: torn rotator cuff, requiring surgery

EE WAS UNLOADING BLOCKS ON FLATBED TRAILER & STEPPED ON A CHAIN BINDER. IT SLID OUT FROM UNDER HIM CAUSING HIS LEGS TO SPLIT & HE THEN FELL OFF THE TRUCK.

EE PLANT SUPER WAS TRYING TO JUMP START ON END LOADER. WHEN HE CONNECTED THE BATTERY TO THE CABLES, THE BATTERY EXPLODED CAUSING BATTERY ACID AND DEBRIS TO GO IN HIS FACE & EYES. HIS EYES WE RE FLUSHED AT THE JOB SITE 4X HE WENT OT HOSPITAL FOR EVALUATION.
