I.NAME WAS TRAM4ING A SUPPLY SCOOP TOWARD THE #2 ROOM ON 1ST LEFT SECTION. WHEN HE WAS AT A LOCATION APPROX 20' FROM THE #3 ROOM. THE FACE WAS BLASTED RESULTING IN SERIOUS INJURY TO I.NAME.
