THE EE WAS MOVING A FRESH WATER PUMP, ONE SIDE OF THE WALKWAY FOR THE PUMP COLLAPSED DUE TO A BOLT BREAKING, CAUSING THE EE TO FALL CATCHING BOTH KNEES BETWEEN THE RAIL AND WALKWAY. THIS CAUSED HIM TO HYPEREXTENT BOTH KNEES.

THE EE WAS ASSISTING TWO OTHER EE S IN SETTING CROSS BRACES TO BE WELDED FOR SAND CHUTES, WHEN A SUPPORT WHICH THE LADDER WAS LEANING AGAISNT TOPPLED CAUSING THE LADDER AND EE TO FALL FROM HEI GHT OF ABOUT 12 FEET. THE EE S RIGHT HEEL LANDEDON THE LADDER WHEN HE FELL.
EE WAS LIFTING THE END OF TELEPHONE POLES TO SLIDE A CHAIN UNDERNEATH TO HOOK IT UP TO THE FRONT END LOADER TO MOVE THEM. WHEN HE BENT OVER TO LIFT THE END OF ONE TELEPHONE POLE, HE FELT HIS L OWER BACK POP.

TWO EE HAD JUST FINISHED CUTTING SOME STEEL ON THE DE WATERING SCREW WITH AN ACETYLENE AND OXYGEN CUTTING TORCH. THE EE HAD BEGUN TO WRAP UP THE CUTTING TORCH HOSES WHEN HE REACHED UP TO HIS C HEST GASPED AND FELL TO THE GROUND THE OTHER EE CHECKED HIS VITAL SIGNS AND BEGAN CPR FOR AN APPARENT HEART ATTACK.
THE EMPLOYEE WAS MOVING BY HAND A SPOOL OF CABLE FROM THE LOADER BUCKET TO A TRUCK, WHEN HE PULLED A MUSCLE AND SPRAINED HIS RIGHT ELBOW. THE EMPLOYEE RECEIVED MEDICAL ATTENTION AND HAS BEEN O FF WORK SINCE THE INJURY. THE EMPLOYEE RETURNED TO WORK MONDAY, 10-25-93, WITH NO RESTRICTIONS.
