Swinging a sledge hammer on some ice buildup over the conveyor base when he lost grip of the hammer, causing his left pinky finger to be smashed in between the hammer and the frame. he reports the bottom half of his fingernail is "popped back" with a "blood blister" noted to the side of the affected finger.

Employee was walking up a hill with a tool box. The tool box broke, causing the employee to fall to the ground.

Employee was climbing down a ladder and a piece of iron fell from the top of the ladder striking him in the left foot.

Former employee told his supervisor that he hurt his right shoulder in a bar fight in March 2010, but he was being treated for right shoulder pain before this because of his job, told supervisor on 6/21/10

Employee was terminated on Monday June 14, 2010. It was after he was terminated that he told his supervisor that he was hosing down a walkway and fell backwards, straining his lower back.

When changing screens on the rotary, the employee's right index finger tip was pinched between the hoisting chain and the rotary. The employee's finger nail was removed and sutured.

Using a pry bar to line up bolt holes on rubber hose for dredge pipe line, pry bar pinched right middle finger, required stitches.

The employee was pushing the plastic fusing machine by hand with a lot of force and felt immediate pain in back of right lower leg, in the calf area. Received medical attention, RX for pain and muscle relaxant and several days of restricted work activity.

THE EE WAS HELPING TO DISMANTLE THE DREDGE WHEN DOING SO HE MOVED A FLANGED PIPE THAT SLID AND TIPPED OUT OF THE SADDLE PINCHING AND SEVERING THE END OF THE EE'S RIGHT THUMB BETWEEN THE PIPE FRAME AND CORNER OF THE DREDGE CABIN. THE EE HAD SURGERY ON 9-6-02 TO PIN BONE ON THE END OF THE THUMB.

EMPLOYEE WAS CHECKIGN A HOSE CONNECTION FROM THE BANK ONT HE PIPELIEN COMING IN FROM THE DREDGE WHEN HE SLIPPED AND FELL CAUSING CONTUSIONS TO HIS RIGHT HAND, HIP, AND SHOULDER.

EMPLOYEE WAS BURNED WHILE BURNING A BRUSH PILE, CAUSING FIRST AND SECOND DEGREE BURNS TO HIS HANDS, ARMS AND FACE.

EE WAS CUTTING FLASHING RUBBER WHEN UTILITY KNIFE SLIPPED AND CUT HIS LEFT ARM, REQUIRING 3 STITCHES.

EMPLOYEE WAS USING ELECTRIC IMPACT TO TIGHTEN THE NUTS ON A COVER PLATE FOR THE SAND SCREW WHEN A PIECE OF STEEL CAME OFF OF THE NUT AND GOT INTO THE CORNER OF THE EMPLOYEES EYE. EMPLOYEE HAD TO HAVE PIECE REMOVED BY A PHYSICIAN.

WHILE ATTEMPTING TO LIFT A CLUTCH PART HE STRAINED HIS LOWER BACK.

THE EE RUBBED A LARGE DEEP BLISTER ON HIS HEEL DUE TO ILL FITTING BOOTS A PREVIOUS MEDICAL CONDITION WAS READERED THE FOOT WITH NO FEELING.
