Welding on rotary in maintenance building, when a spark burnt through his welding glove, causing burn to left thumb. Went to the Dr and received prescription fo anti-biotic.

ON 6/21/01 THE EMPLOYEE DECLINED ANY MEDICAL ATTENTION, THEN ON JULY 11, 2001 HE REQUESTED TO SEE HIS DOCTOR FOR TREATMENT. STATUS CHANGED TO REPORTABLE. THE EMPLOYEE WAS OPERATING A FRONT END LOADER, WHEN HE HIT A BUMP & THE SEAT BOTTOMED OUT JARRING HIS BACK, HIS LEFT ARM ALSO HIT THE SEAT ARM REST WHEN REACHING FOR THE CB RADIO.

THE DOZER OPERATOR WAS KNOCKING A DIRT RIDGE DOWN APPROX. 12'-15' AWAY FROM THE EDGE OF THE BANKWHEN THE BANK CAVED OFF. THE DOZER JUST SLID FORWARD INTO THE LAKE. THE OPERATOR CLIMBED OFF THE BACK OF THE DOZER IN WATER JUST BELOW HIS KNEE AS HE STEPPED ONTO THE BANK. THE DOZER CONTINUED TO SINK INTO THE LAKE, OPERATOR TWISTED LEFT ANKLE.

THE EMPLOYEE WAS HELPING CHANGE A BLOW OUT PIPE ON THE DREDGE PIPE LIEN AND WAS TRYING TO GET THE POWER CABLE OUT OF THE WATER FROM THE WORK BOAT. HE LOST HIS BALANCE AND FELL ON THE 12" PIPE , HITTING HIS CHEST AND FALLING INTO THE LAKE.

THE DECK OF THE WORK BOAT WAS WET AND WHEN HOOKING THE TOW CABLES, THE EMPLOYEE'S FOOT SLIPPED AND HE FELL AGAINST THE DISCHARGE PIPE AND CRACKED TWO (2) RIBS.

TO SECURE HOSE EMPL WERE REPLACI G ON PIPE, THEY WERE USING BAND-IT MACHINE & MATERIAL. AFTER ADJUSTING MATERIAL TO TIGHTEN IT EMPL WAS USING SAW TO CUT EXCESS MATERIAL. DUE TO FLEXIBILITY OF MATERIAL IT STRUCK EMPL'S LEFT PALM LACERATING IT.
