Employee was turning in reverse on electric pallet jack and hit left fifth finger on metal tote behind EE. EE was taken to hospital and laceration was sutured. EE returned to work the following day with modified duties.

Employee was cutting a clamp of a pipe when the knife slipped and cut EE's thumb. Requiring 3 stitches in EE's thumb.

Employee was replacing a vibrator on Dryer. After replacing the vibrator employee looked up to see if vibrator was operating, when a small amount of water with very small amount of acid dripped on employee's forehead and running down into the left eye.

Employee working indoors on dry concrete with good lighting. Employee tripped over EE's own feet dislocating EE's right elbow. Employee was released to full job on the next day but wasn't scheduled to return to normal job until 4/9/2016.

While exiting the area the employee stepped onto a false ceiling and fell. The employee was conscious and alert after the fall and the personal protective equipment in place protected him from serious injury. He was taken to the hospital for evaluation. X-rays were taken of various parts of the employees body and an approx. 1/16" fracture was located in his left heel.
The EE picked up the roll of duct tape and stuck his left hand through the center of the roll and proceeded to pull the tape off of the roll with his right hand, however he did not notice on the backside was a razor blade. Whenever he pulled the tape with his right hand the razor blade made a laceration approx. 3/4" in length on his left middle finger just below the second knuckle.
Employee was driving forklift and whenever he went to turn the wheel he heard a pop in his wrist. He went for a follow up with the doctor on 2/23/2011 and received a cortisone shot in his wrist. There was not any damage to the tendons or bones in the wrist, it is just "painful".

Four employees were in the process of installing metal siding on the side of the plant at the second floor level. A section of overhead PVC pipe that had been secured at the top by rope broke loose, and a section measuring 8" X 5'4" struck the victim in the back of the head. He was wearing a hard hat.

Climbing down from ladder and twisted as they turned away from ladder.
Employee was off loading a tanker, the temp. was very low and ice had formed in part of the tank farm. As he valved off one of his pumps he slipped on a spot of ice causing him to pull his lower back.
EE was using a knife to cut with and had left index finger in the path of the cut.

Employee was working on a spent HF acid line. After washing the line out with water he began to remove part of the line. His feet got wet during the process. Thinking the line had no acid in it he continued to work on it. Later that night his right foot began to burn. The next day at 0700 hrs there was a blister on it. We then sought medical attention.
Employee was taking short cut and stepped over retaining wall, tripped and fell forward catching himself with his left hand.

Employee was removing a piece of broken feed tube from the #2 dryer, his wrench slipped off and his right hand hit the broken end of the feed tube, cutting his right hand.
EE WAS CHECKING BEARINGS ON THE #6 DRYER. HE BRUSHED SOME SAND OFF OF A BEARING WITH HIS RIGHT HAND. WITH THIS MOTION, HE CONTACTED THE DRIVE BELT AND PULLEY. THE MIDDLE FINGER OF HIS RIGHT HAND WAS CAUGHT BY THE PULLEY, BREAKING THE BONE AND CUTTING THE TIP OF THE FINGER.

EE WAS MOVING DRUM (APPROX 140LBS) ONTO A SPOIL CONTAINMENT, THE EE DID NOT HAVE PAIN UNTIL THE NEXT DAY. FROM THEN TIL NOW THE EE HAS HAD SOME BACK PAIN AND NEEDED TO BE CHECKED OUT. THE EE DID NOT LET US KNOW ABOUT THIS UNTIL 9/13/05.
Employee was replacing the drum filter cloth. When he pulled the cloth, his foot slipped and cut his knee on the frame for the drum filter.

THE EE WAS REPLACING A TEE SECTION WITH AN ELBOWON THE ACID LINE. AFTER THE JOB WAS COMPLETED THE EE LEFT THUMB STARTED BURNING
THE EMPLOYEE WAS UNLOADING A PALLET OF MATERIAL WHEN ONE OF FOUR DRUMS TILTED BACKWARDS AGAINST ANOTHER DRUM. THE EMPLOYEE MOVED THE DRUM UPRIGHT AND WHEN THE EMPLOYEE GOT BACK ON THE FORKLIFT HE FELT PAIN IN HIS BACK.

THE EMPLOYEE WAS CLEANING UP PACKAGER AREA AND WENT TO MOVE A DRUM OF TAILINGS MATERIAL AND PULLED HIS LOWER BACK.

THE EE BENT OVER TO LOOK UNDER A LEACH VESSEL AND PULLED A MUSCLE IN HIS BACK.
THE EMPLOYEE WAS DOING HIS DAILY CHECK OF THE DUST COLLECTOR, PART OF THIS IS CHECKING THE LEVELOF COLLECTED MATERIAL. THE EMPLOYEE LIFTED THE LID OF THE BIN BUT COULD NOT SEE THE MATERIAL, SO HE MOVED CLOSER. THE DUSR COLLECTOR PULSATED ANDBLEW FINE MATERIAL ALL OVER HIS FACE AND COVEREDHIS GLASSES.

THE EE WAS CLEANING EXCESS SAND FROM AROUND THE BARRELS AND PALLETS. WHEN THE EE WAS FINISHED CLEANING HE REPOSITIONED THE BARREL AND CAUGHT HIS RIGHT HAND PINKY FINGER BETWEEN THE BARRELS.
THE EE WAS HITTING GLASS FEED TUBE TO AID IN SAND MOVEMENT. THE TUBE BROKE DURING REMOVAL AND DISSASEMPLY WHEN THE EE CUT HIS RIGHT HAND RING FINGER.
EE WAS PUTTING BOLTS BACK INTO BRACE WHICH SECURED THE FLOATING PLATFORM WHEN THE PLATFORM MOVED AND CAUGHT HIS MIDDLE FINGER OF HIS LEFT HAND. THE WIND WAS A FACTOR AND THE WATER MOVEMENT.

EE WAS OPERATING DRILL PRESS TO CUT GLASS PUGS FROM AN UNCLAMPED GLASS CRUIBLE SHARD, WHEN THE CORE BIT CUT THROUGH, IT GRABBED THE SHARP, SPON IT, AND CUT EE ON THE LEFT HAND. HE AWARE OF THE RISK, HAD UNBOLTED AND REMOVED THE VICE USED TOSAFETY HOLD SUCH PIECES AND CHOSE TO HAND-HOLD THE PIECE TO SAVE TIME, INSTEAD OF CLUTTING IT TO SIZE AND PUTTING IT IN THE VICE. EE RECEIVED
EE WAS DESCENDING DOWN THE STAIRS WHEN HE MISSED THE NEXT STEP ON STAIRWAY. THE EE'S RIGHT FOOT WENT TO THE NEXT STEP INSTEAD THUS TURNING HIS ANKLE.
EE WAS SENT TO REPAIR A BROKEN BAND ON A ACID TRANSFER PUMP. EE NOTICED A SPILL ON FLOOR AND CLEANED UP HIS WORK AREA BEFORE AND AFTER THE MAINTENANCE WAS COMPLETED. AT SOME POINT DURING THE M AINTENANCE JOB A SAMLL AMOUNT OF HF OR HCL MIXEDWITH DI WATER PENETRADED HIS LEFT BOTT. TWELVE HOURS LATER AT HOME THE EE WOKE UP ON 3-11-98 AT 400 AM AND HIS LEFT FOOT WAS BURNING.

THE EMPLOYEE WALKED THRU A PACKAGER THAT WASN'T IN USE AT THE TIME. HIS HARD HAT STARTED TO FALL OFF AND HE RAISED UP AND CUT HIS HEAD ON THE LEFT SIDE.
