Mine Utility Operator was mounting a CAT 320-E rock breaker when right ankle turned on the step causing Operator to land on the ground with right ankle. This caused a Bimalleolar fracture to right ankle.

Employee was helping two welder repair crusher during a schedule shutdown. The EE was exposed to the light from the welding operation that was going. The EE had the access to the correct PPE but chose not to wear it. The EE was seen by the local Dr on 11/22/13 and giving perspiration medicine for his eyes. The EE is doing fine and returned to work the same day.

EE driving loaded haul truck downgrade, met pickup and gave room to pass at safe speed. RF wheel of haul truck entered ditch before berm. He braked, but weight pushed truck to a drainage break that forced front wheels to R. Truck crossed berm, fell 30' to toe, landed on top. ee wore seat belt, taken to ER, observed and released unconditionally.

While lifting a jaw crusher toggle block, an electric hoist chain swung and mashed a maintenance mechanic's left thumb against a nearby beam of the crusher frame. The mechanic was guiding the block through the frame when the block cleared the jaw and swung free. The incident resulted in a laceration requiring 9 stitches, loss of thumbnail and broken tip of the bone.
