Fall to the walkway or working surface

Employee was walking out of R142 toward the plant when he slipped on wet concrete and fell onto his (L) shoulder, fracturing his uppermost (L) arm. He was admitted to the hospital and is losing time.

EMPLOYEE WAS CARRYING A 5 X 8 PLYWOOD SIGN UP TWO FLIGHTS OF STAIRS ALONG WITH A CO-WORKER. DUE TO THE STAIRS BEING NARROW, THEY GOT IN A BIND & THE BOARD SHIFTED. THE WEIGHT OF THE BOARD PUT PRESSURE ON HIS LEFT SHOULDER, AT WHICH TIME HIS SHOULDER POPPED.

Fall from machine

EE suffered a fracture to the tops of the bones in his lower left leg causing him to lose time from work. He was attempting to access to come-along so that work could be done on the tail pulley of 18 belt in 16 tunnel. His foot slipped off a rail on which the pan feeder runs and his leg dropped down between a 1"" diameter sucker (support) rod and a return roller.

Employee lost his footing and fell landing on his left wrist, resulting in fracture.

Over-exertion in lifting objects

Employee suffered a left shoulder strain while working on a boiler feed pump. He was lifting a part off of a wood pallet when the incident occurred.

Employee was pulling blinds which required use of an impact wrench, resulting in a strain to his neck. He was treated with over the counter medication. As a result of this injury, he was given an injection for pain on 3/6/06 and resulted in restriction being issued.

Employee was working to remove a slide fromt he liquor trough on the 5th unit in prescipitation when he strained his lower back.

Employee suffered a laceration to his left hand requiring sutures. He was cleaning a 1 1/2 gallon glass sample jar with hot water when it broke. Because he was exerting pressure on the jar to swish the water around in the jar, his hand was forced against the sharp glass

Contact with hot objects or substances

Employee sustained first & some second degree thermal burns to his right foot & ankle. It resulted in restricted work activity. He was at the R-33 high temp digester and was stepping across a shallow (1/2"") trench that contained hot liquor. The toe of his right boot caught the edge of the trench causing the liquor to splash up onto his ankle and down to his foot.

Absorption of radiations, caustics, toxic and noxious substances

Employee was washing the bottom floor of the press bldg. with hose water when he received a splash back. the result was a chemical burn to his left eye which resulted in restrictions.

Struck against stationary object

On 8/1/05, ee was squatting and kneeling down off and on for 4 hours drilling. His knees became sore. He went home and when he awoke on 8/2/05 his knees continued to hurt. He then reported to Alcoa's Medical Dept where he was given Ibuprofen and no restrictions issued. His knees continued to hurt him and on March 29, 2006, he had an MRI. He was placed on restrictions.

Employee was working a ball valve on #6 tray. He pulled the handle with his right arm and as he jerked on the valve he suffered a strain to his right arm. He was issued restrictions.

Employee was setting up a poly pipe in a fusing machine and one of the clamps struck his right hand during the process resulting in a contusion. he was restricted to ""no use of his righr hand"" for two days.

Employee was closing a steam valve in the digestion area when he suffered a strain to his right elbow. As a result of the 2005 injury, he underwent surgery on February 8, 2006. Restrictions were issued. He did not lose any time.

EE WAS ASSISTING CO-WORKERS WHO WERE SETTING SLIDE GATES IN TROUGHS TO ISOLATE 150 PRIMARY FOR CLEANING. WHILE EE WAS RETRIEVING STORED SLIDE, HE STEPPED DOWN AND DUCKED UNDER A CIRCULATING LINE, PLACING HIS LEFT FOOT ON THE LIP OF THE TROUGH. HIS FOOT SLIPPED INTO THE TROUGH, WHICH HAD LIQUOR IN IT. HE SUSTAINED FIRST AND SECOND DEGREE BURNS. RESTRICTIONS WERE ISSUSD.

EE was a passenger in a workhorse scooter when the driver was distracted by a spider hanging from above him. Scooter swerved striking a barrier throwing the 2 individuals forward as the seat popped off upon impact. EE's leg/ankle was caught under the seat and was fractured.

Struck by flying object

EMPLOYEE SUFFERED A FOREIGN BODY TO THE RIGHT EYE THAT REQUIRED THE USE OF PRESCRIPTION MEDICATION. HE WAS WALKING OUT OF THE MACHINE SHOP AND STOPPED TO PICK UP SOME DEBRIS FROM THE GROUND. AS HE PLACED THE ITEM INTO A DUMPSTER, THE WIND BLEW AN UNKNOWN FOREIGN BODY INTO HIS RIGHT EYE.

Employee suffered an abrasion to his left eye which required the use of prescription medication. He was attempting to take a mud measurement and was not aware a gasket had failed. He was treated for caustic in the eye which initially seemed okay. The abrasion was noted the next day.

Employee suffered a laceration to his left little finger which required sutures to close. He was removing pump covers from a pump and when the clips were removed the guard slipped and smashed his hand between guard and beam.

Employee & crew were trying to relocate the scaffold being used when it came into contact with a large chain hanging from the roof of the tank (F7 Thickener Tank Overhaul). While maneuvering the scaffold, the chain fell and employee's (L) hand was caught between the chain and the scaffold handrail. Injury required stitches.

Employee was assisting in moving a file cablinet when strained lower (L) back.

Struck by flying object

Employee was working in R-25 rod mill bldg. & sustained an eye injury that required the use of prescription medication. He received the prescription after he was sent to the hospital for further evaluation. He was lowering his welding hood when a fan blew something into his left eye. There was dust behind him & in front of the fan.

Employee was guiding a press filter screen into the storage rack. As it was being lowered by the crane, it hung up on the opposite end from the employee and when released, struck his (R) thumb fracturing it. Employee was released by the doctor to return to work with no restrictions.l

Employee was using a sledgehammer to close a valve when a metal fragment struck her in the lip. Medical treatment was used to remove it making this case recordable.

Employee sustained a strain to his left knee that required the use of a knee immobilizer. He was walking from the lime area toward the railcar loading area when his knee popped and kept popping on consecutive steps. He was taken to the plant medical facility via plant ambulance and then referred to his own doctor. He returned to work with the knee immobilizer.

Employee suffered a strain to his lower back when opening the lid on #6 spar tank, resulting in medical treatment.

EMPLOYEE WAS WALKING UP A CONCRETE RAMP AND SLIPPED CAUSING HIS KNEE TO POP. THIS RESULTED IN MEDICAL TREATMENT.

Employee was working on #2 meter when restriction caused water and grit to break loose resulting in a chemical burn to both eyes. She was taken to Memorial Medical Center where medical treatment was given.

Employee was stepping down off the front end loader he had been operating and stated his right knee twisted. He was referred to the ER and returned to work with no restrictions but was wearing a knee immobilizer making the case medical treatment.

Employee sustained a four inch laceration above the let eye. Employee was transported to Memorial Medical Center where closed the laceration with sutures. He was attempting to remove top lid of ball check. He was driving a wedge between flange when the wedge popped out and hit him above his left eye.

Employee and crew were removing scaffolds from top of a vessel when a board slid off and struck the employee on the right side of his body. The board had a jagged edge and a large splinter pierced the employee's (R) upper chest and caused pain in his (R) shoulder. Splinter required incision and stitches for removal.

Employee sustained a foreign body to his nose while closing a water fill valve on #19 press. He was using a hammer to close the valve when a piece of metal hit him in the tip of the nose. The foreign body was lodged in the nose and became infected. a surgical instrument had to be used to remove the metal piece.
