Fall from ladders

EE had completed a task on a scaffold landing. Material from the task the ee had preformed on the scaffold caused his ""wet grip"" gloves to become slippery. As the ee feet reached approx 7 feet on the fixed scaffold ladder, it appeared the ee missed his next step down with his left foot. Simultaneously, the ee lost his hand grip on the ladder and fell to ground.

Employee was opening valve on bottom of primary precipitator. Valve was stuck and used excessive force to open valve. After task noticed pain in right wrist.

Employee reported to Medical Dept on 7/8/2010 to report injury stating that he squatted down to go under trough and felt a pop in left knee. Rx medication and work restrictions applied 7/8

Employee reported to Medical Dept 7/5/2010 to report injury. Patient reports that she was swinging sledge hammer to open valve and felt a pull in right shoulder. Rx medication and work restriction applied

Over-exertion in pulling or pushing objects

Employee was assisting another employee to remove a nipple from a 1 inch ball valve. He was using a pipe wrench and as he pulled the pipe wrench toward his body he felt two pops in his shoulder followed by pain. Diagnosed as a dislocated shoulder. Work restrictions applied 5/31/2010.

A precipitation operator was putting a caustic hose on the bottom of tank #70 while standing on a scissor lift. While tapping the hose connection with a 4 lb. hammer, he struck his middle finger that was holding the bleeder.

Employee was swapping a tank and had difficulty operating a valve and strained himself. He felt a strain in his groin area.

Employee was crawling over a pipe and twisted his knee on 10/19/2010. The initial conservative treatment did not resolve the issue and physical therapy was started on 12/6/2010.

Employee was using pry bar to remove screen box and felt pop in lower back with some pain. Felt increased pain and reported to medical dept.

Employee was walking across grating and caught toe in lip of grating which caused her to trip and fall forward to same level; resulting in a contusion to knee.

Fall down stairs

Employee was ascending stairs on side of #20 press in Clarification and her foot slipped and she fell backward. Her foot got caught in the step and she twisted her left knee.

Struck against a moving object

Employee was on top of presss #14 standing on work platform washing inside of press. Press malfunctioned and the door started to drift. Employee braced himself as the door impacted the door frame. Employee was not struck, but the impact jolted his shoulder. Patient has been prescribed Rx meds, restrictions, physical therapy on 7/21.

Employee was closing lid on barge and stepped on one of the corregated ribs and slipped and struck his knee on the surface of the lid causing a contusion to his right knee.

Employee started work on 6/23 at 7:00 pm and was injured at 1:30 am on 6/24. He was using a sledge hammer to open that valve on the bottom of #9 Precipitator tank. He missed the valve handle and hit himself in the right shin with the hammer causing a contusion to shin.

Struck by powered moving object

Employee was in parking lot and walked in front of a vehicle that was traveling at a slow speed and was struck. Employee did not have any significant injuries, but has had problems with knee that has required follow treatment work restrictions started on 10/18/2010.

Employee was using a jackhammer to remove scale for the interior surface of a filter press in Clarification. He got his left hand caught between the jackhammer and the surface of the shell causing a severe contusion to the hand.

Employee noticed pain in right elbow after working graveyard shift on 6/17. Employee did not report to until 6/21.

Employee was operating a valve on the bottom of a precipitation tank and felt a twinge in his back. He continued to work the remainder of the shift. He reported to medical on 6/15 indicating that he was having some pain. He was prescribed prescription medication.

The employee was walking in a pedestrian walkway across main plant roadway and stepped on painted surface of walkway and slipped and fell. Rain had caused walkway to be slippery and the painted surface did not have anti-slip material added to the paint at the time of application.

Employee was operating valves in plate exchanger throughout the day. He awoke the next day and felt soreness and tightness in his lower back and reported to medical on 5/11/2010. He was referred to external doctor and work restrictions were recommended.

Employee was removing a screen from a screen box. He was jerking on the screen to remove it and it jerked his back as it fell to the ground.

Over-exertion in pulling or pushing objects

Employee was hand shaking a stack of screens in the laboratory for particle size analysis of bauxite samples and felt a pop and pain in shoulder. The sample and screens weigh approx 5 - 8 lbs.

Struck against stationary object

Employee was stepping over pipe and bumped her left knee on adjacent pipe. Initially treated conservatively. Patient was not improving so work restrictions applied 5/7/2010.

Employee was using sledge hammer to open valve and valve handle swung up and struck hand and caused a contusion and laceration to back of left hand. No fractures. Work restriction and medication.

Employee was stooping to move under a pipe and as he was getting up, his left foot slipped, twisting his knee. He was treated conservatively at time of injury. Injury did not improve and effective 5/5/2010 he was placed on restrictions and prescribed Rx medication causing case to become recordable on 5/5/2010.

Employee was assisting another employee to lift a portable generator into the back of pickup truck and strained back.

Employee worked on 25A2 tank cleaning screen boxes on Saturday 1/16/10. Reported that he was experiencing pain on Monday morning 1/18/10. When he awoke he had significant back pain and was unable to perform normal duties. He was placed on work restrictions.

Employee's back started hurting on 1/12 and reported to Medical dept on 1/14/10. No specific activity or injury that has caused pain. Work restriction and Rx medication prescribed 1/14/10.

EE was lifting face shield when dust blew into his left eye.

Employee was installing a slide gate in a trough. He removed his goggles to wipe sweat from his goggles. Another employee struck the slide gate and splashed his co-worker with a caustic. Employee was splashed in the eye.

Employee was swapping pumps and screen box gasket failed splashing leg with some caustic.

Employee picked up a control valve with an air hoist and was jerking on the chain to swing the hoist and the chain swung and hit the employee in the left eye tooth. The tooth appeard to be chipped. Employee was referred to dentist. Very minor chip required only minor filing to smooth.

Employee was assisting another employee to move a heater drill and the drill moved unexpectedly and struck the employee in the right arm. Employee was being evaluated in Medical Dept on Tuesday, 11/2/2010 and nurse noticed that abrasion was becoming infected. Rx meds prescribed on 11/2

Employee was walking across grating to inspect a relief valve. The grating was damaged and the employee's leg slipped through the grating. Conservative treatment has not totally resolved the injury. Physical therapy started on 12/1/2010.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was working in the instrument shop in Digestion dept and was exposed to chlorine gas from a spill associated with the filling of the hydrochloric acid tank. The truck driver filling the tank overpressured the tank which caused the pressure relief on the storage tank to relieve. Patient was taken to hospital for evaluation and returned to work the same day.

While removing a 6"" pipe spool with a valve attachment, with another employee, the pipe shifted off balance causing it to lunge upward and pinching employee's left index finger against pipe flange. This caused a laceration to left index finger, requiring sutures.

Employee was working on instrument box and his foot slipped in algae and caused him to fall forward. He struck his finger on an adjacent concrete piling causing right middle finger to hyper-extend resulting in fracture.

Employee went to weather station for daily check and while opening the gate was stung by a wasp/hornet on the scalp. Site of bite has become infected and patient was prescribed Rx medication for the infection.

Working with a co-worker using a jackhammer to loosen scale on gate valve. Jackhammering on top plate of valve. Sliver a metal fractured and became projectile embedding in forearm. Sent to external physician on 6/8 for evaluation and removal of sliver. Suture required to close wound.

Employee stepped onto grating over a sump pit that was 3 feet deep and the grating moved out of position. The employee's left foot slipped into the sump to his calf exposing him to caustic and minor abrasion. 2nd degree burn approx 1 cm.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was exposed to hydrochloric acid (HCL) fumes while working and developed eye, throat and respiratory irritation along with a headache. The exposure resulted from a spill of approximately 100 gallons of HCL at the HCL storage tanks. The employee was working in an adjacent building.

EE was impacting bolts next to another EE who was grinding. His left eye became irritated when he removed his goggles.

Caught in, under or between a moving and a stationary object

EE's small left finger became pinched between a man-way cover and the reinforcement ring of the opening.

Employee was removing filter cloth and became overheated. Employee reported to medical department.

Employee was working on top floor of Precipitation and started to have cramps in arms. Employee took frequent breaks however started feeling worse and reported to Medical Dept at 2:45

Performing normal routines and became overheated and reported to medical. Rested and drank additional fluids and returned to work.

Working in heat and felt weak and drained, with legs cramping at night after work. Reported incident to Medical Dept on 08/12/2010. No treatment returned to work.

Employee was working in Precipitation on top floor and started to feel muscles cramping at 10:00 and took breaks. At 1:45 cramping was more severe and went to Medical dept.

Employee was feeling like his blood pressure was not right and reported to medical to have it checked. Possible heat stress.

Employee was working on top of #3 Boiler in sun and began to feel overheated and started to feel cramps in his leg. Employee got out of sun and went to air conditioned office. Reported to Medical Dept.

Employee was experiencing cramps in his hands and arms during a break after working for 3 hours. He was involved in taking a tank out of service. Employee was working in a hot environment and was drinking while working, but may have gotten dehydrated.

Injured was sick the prior day with gastrointestinal flu. Assigned regular duties on 6/17 and within 2 hours was starting to feel dehydrated. Visited medical dept and assigned work in cool environmental remainder of day. Resumed regular duties next shift.

Employee was working overtime on 5/29 and experienced heat cramps. He did not report to the situation to the Medical Dept until the 5/30. No treatment was provided. He was cleaning heaters.

Employee has experienced a hearing loss a 10dB shift with 25 db impairment. This was confirmed with second test.

Absorption of radiations, caustics, toxic and noxious substances

Skin irritation and secondary infection to tops of hands, skin irriation noted on upper arms. Condition reported to Medical Dept on 3/4/2010. Condition has gotten gradually worse since returning to work in November of 2009.
