All MSHA-reportable accidents at Alcoa World Alumina LLC operations in 2013. Fatalities appear first.
While hydroblasting the airlift on R-45 #80 precipitator tank, the employee went to top floor of the precipitator tanks to monitor progress of the #80 tank. The tank ""burped"" splashing the employee with effluent.
Screen box blew and liquor sprayed all over.
Employee strained shoulder while assisting other employee with adjustment to boom on mobile crane. Conservative treatment was not successful and patient placed on restricted duty on 6/21/2013.

Employee stepped off 18 inch platform and tripped. She felt something pop in foot. Patient rotated off work on 5/30 for 7 days. Returned to work today 6/7 first scheduled shift and was placed on restricted work due to pain in foot.
5/12/2013 employee was pulling on chain to remove belt and felt pain in abdomen. Did not report incident until 6/20/2013. Was being treated by personal care physician prior to reporting to medical dept. Not sure of diagnosis. Placed on restrictions to accommodate until diagnosis complete.
Climbing ladder and foot slipped. Felt pain in shoulder.
Employee opened door and got finger caught in door and door jam.

Walking to department at the beginning of shift and twisted ankle on broken piece of concrete.
Employee was picking up sandbags off pallet and felt a pain in his back.
Descending a two step ladder that crosses over a trough and he stepped on a large piece of hydrate and twisted his ankle.
Employee was washing a press and the pressure of the water hose knocked him off balance and he struck side of the his back against the press causing a contusion.
Employee was ascending stairs to control cab of gantry crane and started to have low back pain. No slip trip or fall at the time.
Employee was assisting with installation of air conditioner and strained shoulder while using a come along to raise unit into place. Patient was placed on restricted duty today 6/28/2013
Fell down the wooden stairway at R84O trailer. There were 3 steps that the employee fell down. He contused his left knee.

Employee struck left elbow while descending small platform. Employee placed on restricted duty on 7/19/2013 to allow healing of elbow.
Employee tripped on upturned grating on 5/14/2013 and fell landing on right knee received a contusion/strain. 5/17/2013 employee was placed on work restrictions.
4/11/2013 employee experienced muscle spasms while at work. No specific task attributed to causing spasm. Reported to Medical Dept 5/13 conservative treatment. 5/17 patient no better so put on restricted duty effective 5/17/2013.
Employee was hammering on valve and felt immediate pain to left side of groin area. Bulge was found over pubic area. Hernia confirmed on 4/10/2013.

Employee was welding in the unit when operations opened an unrelated valve and sprayed the worker with caustic. This resulted in chemical burns to both eyes.
Employee came in contact with grease on 5/29 and reported to Medical Dept on 6/3 that he had a rash on his hands. Patient provided with prescription medication for treatment on 6/3.
Employee was clearing material from C7 vessel and was contacted by hot alumina dust on the top of his safety shoes. He received 2nd degree burn to both small toes.
Employee was handing general mechanic on C7 vessel a tool and was inadvertently contacted by hot alumina. He received 2nd degree burn to left hand and left side of face
Employee was handling rods within a rod mill and got his finger pinched between a roller and a rod.
Employee was grinding a valve and the valve handle broke. The broken handle struck him in the lip and lacerated his upper lip and loosened two teeth.
Employee was training new employees on how to lock a valve. He had a slip in attention and hit his leg with a hammer sustaining a laceration that required 4 sutures to close.
Employee was scanning suction lines when he stepped on the concrete dam and twisted his knee.
Employee stated she was opening a press drain. She used her whole body to move the drain and turned wrong. Was initially a First Aid and was upgraded to Recordable on 3/8/2013
Employee was closing a valve with a hammer and hit his hand on the knocker of the valve.
On 2/5/2013 at approximately 8:30 am, a digestion production employee went to open the liquor valve to 2 rod mill to put in service. The hot liquor was released and hit the ceiling (concrete) and came down onto the employee. The employee received burns to her bottom lip, side of her tongue, shoulder and arm. She was prescribed a mouth wash style burn cream
Employees' knee contacted a trough with hydrate and liquor and he received a burn. Started as a first aid case (1/27/2013) and was changed to a recordable on (1/29/2013) due to infection of original burn.
Employee was working graveyard shift and got hot and shaky and felt dehydrated. Shift started at 7:00 pm on 7/24/2013. Performing normal routine general mechanical work.
Employee was cleaning launder ring on 56-2 tank. He started to feel tired. Reported to medical department and provided with electrolyte replacement fluid. Returned to work.
Employee was swapping pumps #13 and #14 and did not take a break and became overheated.
Employee was using a sledgehammer on a screenbox and became dizzy. Patient was taken to the Medical Dept and provided with electrolyte fluid and rest time in cool environment.

Employee was performing routine maintenance work and started to feel nauseated and muscle cramps followed by chills and headache. Reported to Medical Dept and provide with electrolyte replacement drink and cool environment to rest.
Employee was performing normal routines and became overheated. Employee reported to supervisor and was sent to Medical. Conservative treatment of electrolyte drink and break in cool area. Patient returned to work same shift.
Employee was suffering from the flu and was somewhat dehydrated and the physical exertion and heat at work aggravated this situation and he became more dehydrated and was experiencing cramps in legs and hands.
Employee is showing a confirmed threshold shift in hearing in right ear.
