While hydroblasting the airlift on R-45 #80 precipitator tank, the employee went to top floor of the precipitator tanks to monitor progress of the #80 tank. The tank ""burped"" splashing the employee with effluent.

Screen box blew and liquor sprayed all over.
Employee strained shoulder while assisting other employee with adjustment to boom on mobile crane. Conservative treatment was not successful and patient placed on restricted duty on 6/21/2013.

5/12/2013 employee was pulling on chain to remove belt and felt pain in abdomen. Did not report incident until 6/20/2013. Was being treated by personal care physician prior to reporting to medical dept. Not sure of diagnosis. Placed on restrictions to accommodate until diagnosis complete.

Walking to department at the beginning of shift and twisted ankle on broken piece of concrete.
Employee was picking up sandbags off pallet and felt a pain in his back.

Fell down the wooden stairway at R84O trailer. There were 3 steps that the employee fell down. He contused his left knee.

4/11/2013 employee experienced muscle spasms while at work. No specific task attributed to causing spasm. Reported to Medical Dept 5/13 conservative treatment. 5/17 patient no better so put on restricted duty effective 5/17/2013.

Employee came in contact with grease on 5/29 and reported to Medical Dept on 6/3 that he had a rash on his hands. Patient provided with prescription medication for treatment on 6/3.
Employee was clearing material from C7 vessel and was contacted by hot alumina dust on the top of his safety shoes. He received 2nd degree burn to both small toes.

The hot liquor was released and hit the ceiling (concrete) and came down onto the employee. The employee received burns to her bottom lip, side of her tongue, shoulder and arm. She was prescribed a mouth wash style burn cream
Employees' knee contacted a trough with hydrate and liquor and he received a burn. Started as a first aid case (1/27/2013) and was changed to a recordable on (1/29/2013) due to infection of original burn.
Employee was working graveyard shift and got hot and shaky and felt dehydrated. Shift started at 7:00 pm on 7/24/2013. Performing normal routine general mechanical work.
Employee was cleaning launder ring on 56-2 tank. He started to feel tired. Reported to medical department and provided with electrolyte replacement fluid. Returned to work.

Employee was performing normal routines and became overheated. Employee reported to supervisor and was sent to Medical. Conservative treatment of electrolyte drink and break in cool area. Patient returned to work same shift.
Employee was suffering from the flu and was somewhat dehydrated and the physical exertion and heat at work aggravated this situation and he became more dehydrated and was experiencing cramps in legs and hands.
Employee is showing a confirmed threshold shift in hearing in right ear.
