Over-exertion (Not Elsewhere Classified)

Employee reported to medical dept on 12/11/2009 stating soreness in shoulder. Treated conservatively. Continued working without meds or work restrictions. Diagnosed with torn rotator cuff and scheduled for surgery 1/6/2010. LWD started 1/5/2010.

Bodily reaction, (Not Elsewhere Classified)

Employee reported to work area and bent over to pick up newspaper to read and felt pain in his back and could not move.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was preparing to swap pumps and was exposed to hydrocholric acid mist from a leaking flange that was approximately 15 feet away on adjacent equipment to what he was working. He had difficulty breathing

Absorption of radiations, caustics, toxic and noxious substances

Putting #10 thickener back into service. As they were actuating the valve both employees were looking down into the overflow box to watch the valve to determine if it was opening. On the 5th try the valve burst opened and liquor sprayed upward and directly into the face of injury.

Over-exertion (Not Elsewhere Classified)

Pulling on pipe wrench and strained right shoulder. First aid treatment applied initially. Patient was not getting better so work restrictions applied on 1/8/2010 which changed the case to restricted work.

Fall from ladders

EE slipped on a ladder and left knee hit a beam while holding a beam for a welder. INCIDENT BECAME REPORTABLE WHEN HIS KNEE WAS IMMOBILIZED, ON 1/6/2010, WITH A BRACE.

Hand got caught in pinch point while moving a valve. Pinched 5th digit right hand. Work restrictions 1st applied on 12/15/2009 causing the case to change from first aid to restricted work.

Caught in, under or between a moving and a stationary object

Employee was using a pry bar to open slide gate on bottom of railroad car and the gate slipped open and the bar moved upward and employee caught finger between bar and side of rail car causing laceration to right little finger.

Employee was struck in elbow by valve handle. Patient was treated conservatively(first aid). Patient reported to Medical Dept 1/22/2010 that still having discomfort in elbow. Physician prescribed restrictions on 1/22/2010(Restricted Work).

Over-exertion in pulling or pushing objects

10/31 Opening ball valve that was hard to open. Jerked on handle to try and open. Felt pop in arm. Continued to work and did not report incident until next day. Seen by external physician on 11/1. Reported to plant medical 11/2.

Employee was sitting in skid loader and leaned forward to pick up a bolt on the floor and felt a pull in left side of back. Employee was placed on restricted work effective Oct 13.

Fall to the walkway or working surface

Employee was walking into Area 4 breakroom and slipped and fell on wet concrete striking her right side: elbow, shoulder, hip and hand.

Over-exertion in lifting objects

Employee was lifting valves from one pallet and consolidating them onto another pallet. Felt a twinge in his back

a Digestion General Mechanic was working with a co-worker staging pipe on the ground for assembly and lifting into the overhead pipe rack. The injured and co-worker lifted one section of pipe to align it for assembly with another section when the injured employees foot got stuck and caused him to fall backward with the pipe. The injured fell on his buttocks straining his back.

Struck against stationary object

Employee was changing screen box and struck finger on bolt on 5/28/2009. Incident not reported to plant medical dept until 7/8/2009. While on vacation finger became infected required medical treatment.

Absorption of radiations, caustics, toxic and noxious substances

Employee felt sudden irritation to eye and quickly removed safety glasses and reported to medical. Eye was swollen. Taken to emergency room and diagnosed with insect sting and treated with prescription medication.

Employee was removing pipe from overhead pipe rack and was using a line-up bar to lift pipe past obstacle. The pipe slipped and caused the end of the line-up bar to pull across the palm of his hand lacerating the palm of his hand.

Struck by flying object

Employee was working as the ground safety person in the 510 switch yard as an observer. He noticed something in his eye at break and does not know what may have gotten in his eye. He was not performing work that was dusty.

Employee stood up from table in break room and his left ankle did not support him and he twisted his ankle.

Over-exertion in lifting objects

The rear tire of the three wheeled bicycle the employee was using became flat. Employee strained back while removing the rear wheel in order to repair the tire.

Employee was using a pneumatic air wrench and was pulling on the hose to move the hose. The hose unexpectedly loosened and the employee was pulling toward her face and the air wrench hit her in the mouth breaking a top tooth.

11/23/09 employee reported experiencing discomfort in right wrist when using impact tools. Conservative first aid treatment was started. Patient was not improving so on 1/8/2010 patient was r eferred to specialist and provided Rx medication and placed on work restrictions 1/14/10 hence changing the classification of the case.

Jackhammering material from chute on #16 tripper in Bauxite barn - Bldg 16. Area was hot and employee started to feel ill and stopped and went to medical for rest and fluids. Returned to work.

Employee was walking around with MSHA inspector during Wall to Wall inspection and began to have cramps from heat. Employee was sweating profusely.

Employee was cleaning a screen box in Precipitation and started displaying symptoms of heat stress.

Employee was working in Powerhouse performing maintenance activities.

Area operator in Precipitation working on top floor and got overheated. headache and nausea associated with mild heat stress.

General Mechanic in the Digestion department. Working in area and became overheated. Mild heat stress with muscle cramping and slight nausea.

Employee was performing duties of area operator in Precipitation and became hot and started to have cramps associated with heat exposure.

Employee was performing normal duties and became hot due to heat outside. He was swapping pumps.
