RED LINE CASE unsure of work relatedness, investigation continues employee claims hernia, no specific date of occurrence, states happened in 2014 but did not file injury with Medical department. He did come to medical but it is documented the he wished to speak with union prior to reporting, Employee did not return to file injury report

Struck against stationary object

Tightening a stud on right side of pump, wrench slipped and injured worker struck top of tank with right elbow.

employee was sprayed in the face by chemical causing chemical burns to cheeks and lips

RED LINE CASE - became recordable on 11/23/15 when restrictions were placed, employee was working in shop welding pipe, he reached down to roll pipe on ground to apply wrap and he felt tightness to right elbow

Employee was utilizing the flex lance and he thought that he felt the flex lance build up of pressure in the tube and it pushed the flex lance upward. Water sprayed on his face shield and his face shield fogged, which caused him to temporarily be unable to see. He then felt water pressure hit his right foot.

Employee was hooking up a barge at R52 loading dock, the barge shifted due to wind. He was attempting to board the barge and the barge employee pulled him onto the barge, he felt a pull in his left shoulder.

employee was changing a valve out, he was straddling pipes and when he stepped down he bumped his left knee on pipe and his knee was caught between the pipes causing contusion to the left knee

employee was closing a discharge wheel valve, felt extreme pain in both shoulders

employee was pulling on packing on water ring, used the puller and it broke loose, he lost his footing and fell backward catching self on left hand to break fall, felt pain to left elbow, shoulder and left side of back

RED LINE CASE LATE REPORTING, UNSURE OF INCIDENT DATE AND TIME , WORKED FOR 1 MONTH BEFORE SEEKING MEDICAL ATTENTION states that while closing a liquor valve he missed the valve with the sledgehammer and felt pain in right shoulder

ee was repacking a pump, the packing gland was stuck under the shaft he applied pressure to pull it out and felt right wrist pop and pain to right wrist

packing was leaking on valve, caustic sprayed out in face and ran down goggles and into left eye causing chemical burn

Employee states that he was beating on multiple press feed valves and repeatedly tried to seat valves as daily routine, pain in elbow started 6 months ago.

EE was pulling a 6 foot rolling stand, EE fell and stand fell onto low abdomen. has pain to tailbone.

Employee pinched finger between railing and pipework. Per email from reporting party, EE was changing out the bag on the number 3 zimpro using a pipe wrench to hold bag up, while coworker, (new to job) started loosening the fitting. Injured EE's finger was caught between handrail and pipe wrench.

Employee was tightening valve packing, wrench slipped off nut and crushed finger between valve and wrench causing a tuft fracture 4th digit left hand.

Employee was closing a discharge valve, he felt a pull in low back.

We had to move the old valves to the other side where the crane could fly them out. During the process of moving the old valve over the new one, it got caught. Two people let go but the third person didn't. The valve shifted in direction of the heater and pinched his hand.

On Tuesday 4/14/15 at a tank roof a worker was gathering tools and stepped over a pipe with his left foot near a ""rafter"", he then stepped with his right leg. His left foot was still held in place by the rafter causing his left knee to twist causing discomfort.

RED LINE CASE. Injury date was 3/9/15 did not become restricted until 3/13/2015. Closing blow off valve, was loosening up a valve with a 8 lb hammer, felt pain in right elbow.

Employee was rolling a pump and finger caught between shiv and belt causing nail avulsion, laceration and left third finger fracture.

Fall from machine

Employee stepped off rail car to platform and foot slipped and missed platform. Fell onto buttocks and left leg fell into open space between rail car and platform causing contusion and abrasion to knee.

Re-occurrence of back pain from initial back injury 12-2-2014. States no specific injury but turning wheel valves and hard T valves hurt irritated back condition.

employee was welding on his knees, felt stinging on his knees and discovered that he was kneeling in caustic residue, minimal burn to left knee, 2nd degree burn to right knee

Employee was caustic cleaning with a pry bay to remove scale, goggles became fogged, took goggles off to wipe them off, was wearing safety glasses and pry bar fell into trench , and caustic splattere under safety glasses causing burn to left eye
