EE was working on the seko foam supression building. He was allegedly installing steel siding and experienced back pain. He was taken off work on 11/27/07 by a doctor. He has has a history of back problems.

EE was allegedly taking a shower after his work shift and he slipped on the shower room floor causing his foot to go sideways striking a locker. He went to a dr on 10-10-07 who took him off work. The x ray is negative he has an ankle sprain.
EMPLOYEE WAS STEAM CLEANING IN THE PROCESS ROOM WHEN HE STUMBLED AND RAN STEAM ACROSS RIGHT FOOT.
Employee claims he reached down to pick up small piece of angle iron off the floor and felt pain in his lower back.
The employee was reaching for a powerpack as a co-worker handed it down to him. He claims he pulled his right shoulder when the other employee dropped the powerpack to him. He reported it, but thought it would be OK. However, the pain has persisted. An MRI on 8/13/07 showed he has had some trauma in the shoulder and he was given 2 shots.
Emp. stated while driving elevator chain pin back to make clearance on chain link section, he felt pain in lower back. Pushing an approx. 30 lb. object with a twisting motion.
Employees were charging a ball mill and setting a drum full of balls into a cradle dump when the drum slipped out of choker and struck the employee. He ended up needing surgery on 4-06-07 and this became a lost time on 4-09-06
Employee was using a pipe to break chunks in the waste dust pump and when the chunk broke his hand went forward and hit the side of the port causing a fractured thumb.
Employee stated, while climbing a ladder, he felt something pull in his left knee. First lost time day 4/12/2007.
Emp stated he was lifting a piece of lumber from the 5th floor of the RSP Tower to the 5 1/2 floor. When he slid his hand down the board to get a better grip, a splinter from the wood punctured the left palm. Emp was wearing leather gloves at the time of injury. His first lost time day is 2/8/07.
An employee with 36 years experience currently assigned with the Repair Department as an oiler was carrying a 5 gallon bucket of cleaning solvent up a set of stairs by the tire feed conveyor. The employee heard a ""funny popping sound"" followed by extreme pain from his left knee radiating to his ankle. Orthopedic Specialist diagnosed the injury as a left knee torn Miniscus.
Employee reported he was walking and turned sharply and slipped on a piece of clinker causing him to fall on his left shoulder. First Lost time day is 4/3/2007.
Associate pushing wheel barrow, slipped on mill grinding balls and fell, strained lower back.
EE was given the job of drilling three 1/2"" holes in the coller fan ducting for number 1 fan. He drilled 2 holes with no problem, but the bit bound up on the third hole and he lost his grip and the drill handle spun hitting his hand.
Employee was desending a staircase, when his foot slipped, causing him to fall to the ground. He dislocated his left foot in the process.
Employee was stepping off a mill to the catwalk and stepped on an airhose. He twisted his left knee. Pain persisted so an MRI was done in April, a minor tear was found. Employee has personal health issues so surgery was put off until 5/3/07 (just prior to the shifts long weekend). Employee was allowed to return to work with restrictions.
While removing hood from elevator bucket area, wearing leather gloves, pinched and cut right middle finger requiring sutures.
Employee was allegedly setting up a 20' extension ladder and it started to slide sideways and he tried to stop it and injured his rt arm. He had a medical procedure done on 12-20-07. This incident became reportable on 12-20-07. Until then first aid only.
Employee cut right ring finger while cutting vacuum hose. Injury required 6 stitches.
Employee was attempting to remove couplings and a shaft from an FK Pump Drive Bearing. The shaft slipped from the employee's grasp and smashed the employee's left index finger.

EE's shift was having problems with #1 finish mill. EE went out to see what was wrong with the finish mill. He found that the tipping valve was stuck open coming from dust collector. When he pushed the valve arm down, the counter weight slid down the handle and hit his right hand cutting it.
Worker was attempting to reposition forks. (Forks are hard to move). He let it down with his right hand to push over with his foot, it swung back and smashed finger on left hand.
Emp. reported that while shoveling material off the roof of KB2300 compressor roof, a wind picked up material and blew up into his face getting dust into the right eye. This a reportable only. He did recieve a prescription from the doctor for the eye.
Employee tripped while walking carrying material for the jobsite. Material fell with his body to the ground hitting his right hand injuring ring and pinky fingers. Employee went to the hospital for treatment, x-rays & employee had fractured both fingers and received stitches in the nail area of both fingers.
Employee was lifting railroad track steel up over top of head and pulled muscles in back.
He was assigned the task of installing cardox ports on a bin in the rock belts. He set the cardox port on top of a cabinet to get it out of the way and it slipped off striking his second finger on his left hand causing a finger tip fracture.
Employee was cleaning a spill from a drum and got hazardous waste on his pants & shoes.
This is a reportable illness, the employee was diagnised with Carpal Tunnel on 3/30/2007. Skin grafts from a previous injury contributes to the current condition. We were notified on 4/2/07 of the diagnosis
Bilateral Carpal Tunnel due to repetitive motion (left and right wrist)
During lunch the employee began having chest pains, his fellow employees notified the supervisor and safety manager immediately and the employee was transported to the local hospital. It was confirmed by the hospital that the employee was having a heart attack.
