Accident type, without injuries

Kiln #1 baghouse ductwork unplanned ignition. The pressure resulting caused damage.

Caught in, under or between a moving and a stationary object

EE was attempting to unplug a triple gate at the coal mill with no success. EE then introduced EE's hand into the triple gate. The gate activated, nipping the tip of EE's right middle finger.

Fall to the walkway or working surface

Employee was working on housekeeping around the plant. While working with the vac-truck operator employee was giving hand signals to the driver during the backing up process. While doing this their feet slipped and they began to fall. during this process their right leg became entangled underneath them and they broke their leg.

Caught in, under or between a moving and a stationary object

Employee was removing roll over protective structures of wheel loader, the right side was clear when the left side was hanging from crane ROPS and hung up on filter housing, they put a hand on the cab to see what was stopping it when they touched the cab the ROPS shifted pinching their finger against the cab.

Fall to the walkway or working surface

While walking they slipped on ice and bumped a knee.

Stepped off of forklift onto uneven ground causing a minor fracture to left foot injured employee is on restricted duty

Contact with hot objects or substances

While inspecting a port on the tower the employee got hot dust in their face causing an eye abrasion and 2nd degree burns to face.

Fall from machine

Employee was climbing into the cab of the bulldozer EE operates when EE's foot slipped off the step and EE fell injuring EE's ankle in the process.

EE was walking from EE's office to the front break room for the morning meeting. EE heard some voices from the direction of the break room. EE turned EE's head to see who was speaking. At this time, EE's foot went into a depression in the walkway, causing EE's knee to twist awkwardly. EE felt something pop in EE's knee and immediately began to feel pain

EE was washing out the raw mill posimetric chute with a water lance. The water hose became entangled with the door, causing the door to begin closing. The door struck the employee's right right wrist resulting in a contusion. The injury became reportable on 11/18/25 when the doctor performed professional medical treatment during a follow up visit.

Absorption of radiations, caustics, toxic and noxious substances

While cleaning a chute with water the individual had wet high alkali material get behind EE's safety glasses and into EE's eyes causing minor chemical burns

The employee was using a knife to cut a hose (not the company approved safety blade). While cutting the hose the employee cut EE's left forearm.

Fall from ladders

While the employee was climbing down a fixed ladder EE slipped on the 3rd rung from the bottom and landed awkwardly on EE's left ankle.

Struck by flying object

WHILE ATTEMPTING TO MOVE A VALVE HANDLE THE TOOL SLIPPED AND STRUCK THEM IN THE CHIN

Employee was grinding a beam when it grabbed on the beam and the hand holding the handle slipped off. The grinder slipped back into their wrist as they came off the trigger with their other hand. The disc caught their left forearm and required 4 stitches. EE was wearing proper PPE and all guards were in place.

While changing rolls employee had a piece of clinker dust get behind EE's safety glasses and in EE's eye. EE was given antibiotic eye drops after removal of particle on 03/03/2025

Fall from scaffolds, walkways, platforms

EE was assigned to build a platform and hopper. EE was wearing a harness but did not tie off and fell through the unfinished platform. The fall was approximately 13 feet. Injuries include a cut about the eye, wrist fracture and teeth knocked loose, and a cut on lip.

Struck by flying object

While walking across the parking lot, wind blew material into the miners eye.
