Caught in, under or between running or meshing objects

While trying to remove blockage and improve product flow an employee reached up and inserted hand into an energized rotary feeder. It resulted in the partial amputation of three fingers of the right hand.

EE was struck by metal shard, which penetrated the Left Forearm, initially was treated with first aid only on 08/17/2018. Later had the fragment surgically removed on 08/28/2018.

Employee had gotten off of forklift to move steel plate back on forks that had fell off and felt pain in right shoulder and bicep area.

Employee was stepping from stairs to the landing and stepped onto a ball of clinker, spraining their right ankle.

Employee was inspecting fire extinguishers. Employee walked inside of train locomotive and stepped into floor hatch, injuring left leg. The floor hatch was left open by employees to allow heat to get to air lines. Employee did not use barricade or signage.

Employee dropped a sprocket when removing it from a shaft. The left little finger was pinched between the sprocket and the previously removed guard that was laying on the floor.

EE was crossing a gantry from work platform to trailer. Gantry dropped slightly, causing EE to trip.

Employee was reinstalling guard over tail traction and mashed hand between the lifting handle and the other guard that was already in place.

ee was using a pipe to hit inside of a chute to dislodge a diverter gate. The gate swung open with force, hitting the pipe that the ee was using. The pipe struck the ee on the shoulder and chest area causing a shoulder sprain and chest contusion.

Ee stepped backwards and was struck by the railing of a locomotive.

EMPLOYEE WAS CLEANING SENSOR FOR MIX TANK. SWEAT CAUSED STREAKING ON RESPIRATOR MASK AND LOST SIGHT OF WHERE TO STEP. MISSTEPPED AND FOOT/ANKLE POPPED.

Employee had thrown track switch for rail cars and felt pain in left arm.

Employee got thumb pinched on door of primary crusher while trying to open it.

Employee was in feed chute removing an eye bolt from a plate with a wrench. The wrench slipped and struck employee on right side of face.

Employee was unplugging wet buildup at the Kiln 3 #16 dust screw. EE received alkali burns to EE's hands.

Struck by rolling or sliding object

The employee was assisting forklift operator set a 1-1/4"" x 12' metal plate on a tie block so the operator could readjust the metal plate. The metal plate slid off of tie block onto injured employee's foot.

Employee was removing scaffold board in 4th stage cyclone of the preheater vessel, when EE tripped over hose while carrying scaffold board, injuring EE's left knee.

Employee stated ee stepped off a ladder with weight on right leg. EE foot slipped and then grabbed and ee felt the top of calf pop.

Employee was repositioning dust mask and lost grip. Mask popped against face causing dust to go under safety glasses and enter eye.

Employee received restrictions due to sustaining 2nd degree burns from hot material contacting right shin. EE poked material that had plugged a chute and it came out towards ee rather than staying in the chute.

EE smashed left finger with jackhammer and metal gate while removing material buildup.

EE'S LEFT HAND PINKY FINGER, SLAMMED IN DOOR. CAUSED LACERATION, REQUIRED 3 STITCHES.

Injury Crush Avulsion injury to the pad of the left index finger. Shift supervisor was working on a diverter gate and failed to remove residual energy in the diverter gate causing injury to finger.

Employee had cut right arm on a burr located on stem at double dump.

Struck by falling object

Employee was tying rebar with tie wire and went to cut a piece above their head and it fell and struck them in the right eye. The employee had removed their safety glasses to clean them prior to cutting wire and did not put them back on before performing the task.

Struck by flying object

While cleaning a cooler tipping gate, an air cannon discharged and blew dust into EE's left eye. Foreign material was removed and a prescription was filled. Employee was wearing safety glasses.

Employee was cutting plate with a grinder ans while taking off a little to fit into place the grinder came off the steel and struck employee in the left thumb.

Employee was getting into position to cut bar grating and while getting into position the employee cut their leg on a protruding piece of steel.

Miner suffered impact to left thumb.
