Fall to lower level, (Not Elsewhere Classified)

Employee removed lid from manhole to pump out water from scales. While waiting on water removal process to be completed, EE picked up an air hose and began walking backwards to remove slack from the hose and stepped into the manhole causing employee to partially fall into opening striking lower back

Struck by falling object

While removing slide above screw conveyor material inside the hopper moved and dust came out of the gap left from the slide removal and got into employees eyes

Over-exertion (Not Elsewhere Classified)

On February 4, 2026, an ACME field employee reported to EE's foreman that EE's right forearm felt strained but did not believe medical attention was needed. EE later requested medical attention, stating EE's forearm began hurting after using an air hammer for two hours removing excess concrete.

Struck by falling object

While moving a long handled scraper the handle hit a beam overhead that dropped dust on top of employee and EE got some in EE's left eye

Struck by flying object

While removing buckets from a bucket elevator employee got something in their eye. Eye doctor removed and prescribed antibiotic eye drops

Struck by flying object

Employee was working near the doorway at the alkali spray tower dump box when wind blew dust under EE's safety glasses. This caused EE's left eye to become irritated. This did not become reportable until the employee went to the doctor on 1/9/2026, when the doctor prescribed eyedrops.
