Over-exertion in wielding or throwing objects

A 64 year old Miner with over 26 years experience was doing a hoist resocket on a dragline. As he raised the sledge hammer to knock out the wedge from the socket he felt a popping sensation in his right bicep. The employee visited several doctors getting a second opinion and missed his first day of work on 11-5-2012

Over-exertion in lifting objects

On 8/28/2012 employee was shoveling gypsum, which weighs approximately 15 lbs per shovel into a loader buckets. During this time, the employee seemed to pull/sprain a muscle or tendon in his elbow region. It seems to be a very minor injury. Employee did return to work during the ten day, however, went back on disablity.

Struck by falling object

Employee was installing a 500 lb. raw mill door with another worker. After taking it off the hoist onto door frame, he attempted to use a pry bar to line up the bolt on door hinge. As he was prying it, the door slipped off door frame and fell onto top of foot.

Over-exertion (Not Elsewhere Classified)

While employee was walking down stairs, he missed a step and partially fell off balance stretching right knee forward. After catching himself with handrails, he noticed severe pain in knee.

Struck against stationary object

Employee was tightening a bolt on small vibrator mounted on the gypsum hopper. He used two adjustable wrenches to tighten the bolt, one on the top and one on the bottom to prevent the bolt from moving while tightening. He used both hands to push on the bottom wrench when it slipped off. This caused his hand to jerk forward striking metal hopper with top of hand.

Struck against stationary object

While water blasting in riser located at preheater tower, material fell down on tool jerking it upward. His hand was pushed upward by the handle of the water blaster tool into an air cannon nearby that smashed his finger.

Conveyor rolled back catching employees leg between back of chute and conveyor.

Struck against stationary object

The injured employee was cleaning around the raw material conveyor belt using a shovel when he extended to far and struck his left wrist against the conveyor frame causing a fracture.

Employee was removing a broken piece of a hammer handle from the hammer section using a bolt and another hammer. He struck the bolt twice with the hammer in order to remove the broken handle section when the third strike he hit his left thumb causing impact damage to the thumb.
