Fall onto or against objects

EE was cleaning ash chute in Raw Mill 2 with an air lance working through and access door. Material came through door, knocking EE down against the handrail. EE sustained a dislocated right shoulder.

Struck by flying object

Mobile mechanic was in the process of removing a front end loader bucket tooth which was seized on the bracket so mechanic used a sledgehammer to remove the tooth, when a piece of metal from tooth embedded on left shin.

Struck by rolling or sliding object

Placed hand on trolley rail the pan conveyor then rolled over middle finger on right hand.

Caught in, under or between a moving and a stationary object

Employee was operating a McCloskey ST110 Portable Stacker via remote control and ran over ee left foot with the left track of the machine, breaking 2nd and 3rd metatarsal bones in foot.

Struck by falling object

Employee dropped a piece of square tubing on EE's right foot resulting in a fracture to the small toe.

Over-exertion in lifting objects

On 11-15-2018, employee reported pain in lower back while lifting and carrying crusher liners. On 11-16-2018 employee was medically evaluated and put on restricted duty.

Struck against stationary object

Employee was on back inspecting the inside of the sizing screen. As employee was crawling on back employee rested back on a dam causing employee pain to back. Employee was taken to clinic on 6/29/2018 and returned to full duty. On a follow up visit to the clinic on 7/12/2018 employee was put on restricted duty.

Struck against stationary object

Employee sustained laceration to the left ring finger from contact with a sharp object. Laceration required 5 stitches.

Struck by flying object

EE was using a sledge hammer to drive a pin in. The sledge hammer was harden steel. The pin is soft blow material. As the pin seated the hammer made contact with the area around the pin. This resulted in a small piece of metal entering ee's lip. When the metal was removed it required 2 stitches.

Caught in, under or between a moving and a stationary object

Maintenance mechanic was performing repairs to a pneumatic cylinder and pinched left ring finger.

Struck by flying object

Employee was removing hydraulic line on gate actuator and nut on hydraulic hose hit employee in the mouth and chipped the Left Front Tooth. No other injury sustained, No Restrictions. Came back to work that day. Needs to get tooth repaired on 5/15/2018.

Struck by flying object

Employee got some dust in left eye and eye was irritated. Took to Urgent care clinic and eye drops and tetanus shot was prescribed to prevent infect. No days off or restrictions given. Employee back to full duty that day
