Over-exertion (Not Elsewhere Classified)

Miner task was to remove and replace steel Grate Decking (1' x39""). EE set down the old decking and foot slipped into the opening. The miner was wearing fall protection.

Fall from machine

EE was dismounting the 4600 Drag Line. When EE stepped on the second step from the top EE's foot slipped off. This caused EE to loose EE's handhold and fall to the ground. This resulted in a fracture to the LEFT hip. EE fell approximately 4'.

Bodily reaction, (Not Elsewhere Classified)

Employee was exiting a diesel truck. As they stood and twisted to exit the employee felt something in their L thigh. Employee was sent for diagnosis. Dr.stated no treatment needed. EE reported pain persisted. Sent for follow ups. EE missed first day of work on July 5, 2017.

Caught in, under or between a moving and a stationary object

While dismantling a hopper, employee was injured. While cutting loose a beam from the hopper, the hopper sank into the soft ground catching their left foot and injuring the ankle. The parts being cut loose were being supported by a forklift to limit incidents of falling material. The soft ground caused by recent rain fall was the prime factor in this incident.

Employee caught right hand in cable sheave while hoisting a valve

Struck against a moving object

Employee was operating a haul truck and the bed of the truck was lifted by loader when the bucket pushed down material at the end of the bed. Bed lifted and came down shaking the truck.

Fall from ladders

Employee was descending a fixed ladder in the Finish Mill and lost grip and fell to the decking below. Employee was taken to a doctor for evaluation where X-rays and MRI were performed. No fractures or significant injuries were sustained other than bruising. Employee was placed on light duty.

Employee was working on changing a bearing in the plant. Their foot slipped and they rolled their ankle resulting in a sprain. Employee was placed on light duty.

Struck by falling object

Employee and co-worker had cut a piece of scroll plate out, and while laying it down, the plate caught the top of employee's glove and hit their hand and wrist as they laid it down, bruising their left hand/wrist area.

Employee was working on Portable Crusher and was struck by a blunt object resulting in an abrasion and bruise to the lower back and pain to EE's torso.

Fall to the walkway or working surface

Two employees were pulling electrical cable when the rope broke. One employee lost their footing and fell backwards on to their back side.

Employee was coming from the cement silos heading to the car shaker walking across the railroad tracks when EE twisted the left knee. EE was taken to the company clinic where was seen by a doctor and diagnosed with Sprain of unspecified site of left knee and prescribed Physical therapy. Weather conditions and visibility were good at the time of the event.

A maintenance employee supporting production to ball Finish Mill #6 was the flagman situated at the top of the mill giving instructions to the crane operator to place the grinding media drums in the mill door. EE placed hand between the drum and the device used to carry them, the drum shifted and smashed left middle finger.

The employee was walking to the working station and tripped and fell over a box, landing on left leg on the side of the knee.

Employee was holding a crimped hose in employee's hand and accidentally let go of the hose and the hose opened up and hit employee in the face causing a small laceration on the inside of employee's top lip requiring a few stitches.

One electrician and one helper were working on a piece of mining cable. The electrician was stripping the mining cable with a razor knife and the helper was helping to hold the cable. The knife slipped off of the cable and cut the helper on the right forearm.

Over-exertion (Not Elsewhere Classified)

Employee states that EE was working on EE's knees for about 15 minutes adding water to the plant generator when EE started having pain/swelling to the back of EE's left knee. On 01/31/17 EE went for a follow up visit with the Dr. and was prescribed Physical Therapy, the event happened a week earlier and no treatment prescribed.
