Accident type, without injuries

A contractor was entrapped on the Clinker Silo #2 Elevator for more than 30 minutes due to an electrical problem with the elevator. The contract employee was safe and was not in any danger. The elevator contractor was called to repair the elevator.

Caught in, under or between a moving and a stationary object

Employee was checking the gearbox to pinion alignment on a ball mill. EE was turning the coupling when the stored energy in the drive system caused the coupling to counter-rotate resulting in EE's hand slipping. The right pinky finger was pinched between the 2 coupling hubs causing a laceration and fractured and displaced bone.

Over-exertion in lifting objects

EE was attempting to lift a safe rack when EE felt pain in EE's left shoulder resulting in restricted duty.

Employee was climbing caged ladder at conveyor 18. EE was on the last rung when EE heard a pop sound and pain on right bicep.

Was operating Excavator. Got out and walked over to talk to dozer operator. As EE was walking over, EE stepped in a hole and dislocated their ankle.

Employee stated that was working with a driller to put the hose for the sump pump into the lake, when EE felt a sprain/strain type feeling in their back. When positioning the hose, body position can become awkward requiring twisting.

Caught in, under or between a moving and a stationary object

While completing a bucket exchange, EE was inserting a pin into the hitch extension. A finger became pinched between the head of the pin and the hitch extension. This resulted in a nondisplaced fracture of the distal phalanx of the right ring finger.

Struck against stationary object

Maintenance Employee was installing sheet metal roofing on a structure and caught corner of a piece of roofing with EE's right forearm resulting in a 3 Inch laceration that required 9 stitches to close the wound. No lost time or work restrictions.
