The incident happened while trying to weld a flatbar, 6"" by 36"", to the inside of the hopper. While placing the flatbar, right before lowering his welding hood to start welding, some material gave away under his feet. He slipped backwards and struck himself with the flatbar right above his right eye causing a cut on his right eyebrow.

While driving a manlift employee hit a small ditch bounced lost balance and his right foot slept out of the basket impacting the toe guardsand cutting his calf
EE (Contractor: Mobile Contract Services LLC representing Navman Wireless) was climbing down from cab of a Cat Grader while inspecting for install of telematics GPS equipment, and on the ground, he stepped back and tripped on grader blade that was parked at an angle, falling backwards. While attempting to catch his fall, he broke his left wrist.
Maintenance mechanic was tightening bottom base nut on finish mill dust collector fan when elbow struck the floor unexpectedly. The nut was located in a tight place under motor and mechanic had to lay on floor to reach the nut.
EE was in the process of attaching two 1"" pieces on conduit together using a coupling. EE was holding a pipe wrench with EE's left hand holding the piece of pipe from rotating and treading the piece on the right using a pipe wrench also. While holding the pipe wrench with EE's left hand EE heard a pop from EE's left shoulder.
On 8/21 employee sustained a puncture wound while using a hammer and punch. On 8/25 he was complaining that there was a piece of metal in his left hand and taken to the doctor. The hand was x-rayed and a piece of metal was. He was sent back to work with NO Restrictions. On 9/2 he had the piece of metal surgically removed and placed on restrictions with limited hand use.
Employee was setting a panel on a sand tank when it slid off and pinched his right middle finger.
EE states he was lifting a 5 gal bucket of oil above his head and pulled left lower back.
Employee was closing the doors on a rail car. Lever used sprung back and pinched employee's left hand middle finger,
EE was troubleshooting an FK Pump when the actuating counterweigh took an unexpected upward thrust and then downward catching EE right hand middle finger between the counterweight and the discharge pipe flange
EE was assisting a crew of maintenance mechanics replacing a link in an elevator chain using a pin driver to drive out a broken pin. While striking the pin driver a sliver of metal became airborne and struck EE on his right cheek causing a cut.
Employee was lifting the lid of a metal container and the lid hit his forehead and caused a laceration on the left side of his head that needed 7 staples. He was released to full duty.
EE was operating a forklift at the cement plant finish mill area during a ball mill changeover. While moving a stack of pallets from finish mill area to storage area outside, he accidentally ran over a wooden pallet with rear wheel of forklift. When the pallet broke under the weight of wheel, a piece of wood flew in the air inside the cab and struck EE in mouth.
Removing liners from a ball mill a liner was stuck, he struck the liner with a sledge hammer to dislodge it causing it to shatter and a piece of metal hit him in the lower right leg shin area causing a laceration and a piece of metal in right leg.
While tightening bolts on a gearbox housing ee was positioning the hydraulic wrench on bolt 8 out of 16. While placing in position the hydraulic unit moved slightly catching ee left thumb between the hydraulic wrench and the gearbox housing causing a cut that required 4 stitches.
The task was re-positioning of a metal hopper preparing it for welding. During this transition the metal hopper tilted backward toward the forklift mast. The EE placed his hand between the metal hopper and the forklift mast to stabilize the metal hopper. Thus causing it to pinch the tip of his middle finger of his left hand.
Employee was pulling down the Electrical Disconnect Arm at the Dredge/Booster Pump Substation. When he had the arm of the booster pump almost all the way down he lost his grip on the handle of the disconnect arm and the spring tension in the arm threw the arm upwards hitting him in the face just below the nose. Employee sustained a fractured nose and lacerations to his face.
Employee suffered from a heat related illness while using a 1"" water hose at the sand plant.
