A fall of supported roof was found during weekly exam in an abandoned area of the mine. The fall was located at #66 break on south mains #5 & #6 entry. The fall measured 20' x 90' x unknown height.

A fall of supported roof was found during weekly exam in an abandoned area of the mine. The fall was located on No. 8 panel #3 crosscut off south mains. The fall measured 20'W x 300'L x unknown height.

A fall of supported roof has occurred out by the active section on #6 belt entry in by spad station #1493. The fall measured approx 20' by 60' by 10' high. No injuries resulted, however the conveyer belt was damaged.

A FALL OF SUPPORTED ROOF OCCURRED ON A PANEL, 1 NORTH MAINS, 001 SECTION AT BREAK 27 IN THE INTERSECTION OF #3 ENTRY. NO INJURIES RESULTED, VENTILATION WAS NOT DISRUPTED, NO EQUIPMENT WAS INVOLVED. THE FALL MEASURED 20'DEEP X 45'WIDE X 5 1/2'HIGH. THE AREA WAS CRIBBED UP PRIOR TO THE FALL.

A fall of supported roof was found by the mine examiner and MSHA inspector in the #1 entry left side return on the 002 MMU. The fall is located at 39 crosscut adjacent to No. 5 belt. No injuries resulted and no equipment was damaged. The fall measured 20' w by 40' by 6-8' high.

An unintentional roof fall was discovered int he #7 entry 3 west mains 2 breaks outby spad 651. estimated size 6'h x 20'w x 90'l. area had been supported with 6' torque tension bolts. no injuries, no equipment caught, ventilation was not interrupted.

A FALL OF SUPPORTED ROOF WAS FOUND BY THE MINE EXAMINER AND MSHA INSPECTOR IN THE WEST MAINS NO 2 RETURN AIR COURSE. THE FALL WAS IN THE ENTRY ADJACENT TO SPAD STATION #8641. NO INJURIES OCCURRED AND NO EQUIPMENT WAS DAMAGED. THE FALL MEASURED 200' LONG X 20' WIDE X 8' HIGH.

A FRICTION FIRE OCCURED WHEN A BELT BROKE & ENTANGLED ITSELF IN THE 8A HEAD DRIVE POWER ROLLERS. MSHA INSPECTOR WAS IN THE MINE AT THE TIME OF THE INCIDENT. IT COULD NOT BE DETERMINED THE DURATION OF THE FIRE. DELUGE SYSTEM PUT IT OUT.

A FALL OF SUPPORTED ROOF OCCURRED AT SPAD STATION NO. 447 IN THE #3 ENTRY ON #4 BELT ON SOUTH MAINS. NO INJURIES RESULTED FROM THIS FALL, NO EQUIPMENT WAS DAMAGED, VENTILATION WAS NOT IMPAIRED. THE FALL MEASURED APPROX. 18' X 20' X 7' HIGH.

A fall of supported roof occurred on the 001 section, 15th crosscut, no. 6 panel, in entries no. 1,2,3,5,6,& 7. The fall was appx. 10-18' thick, rib to rib. No injuries occurred and no equipment was damaged.

An unintentional rock fall was discovered in #5 entry at #11 break. Size of fall estimated: 4 1/2'H x 20'W x 20'L. Area had been supported with 4' fully grouted resin bolts with additional support consisting of 10' cable bolts and cribbs. No injuries, no equipment caught.

A ROCK FALL OCCURRED INT HE INTERSECTION OF #4 ENTRY AT CORSSCUT #89 ON THE 002 SECTION. THE INTERSECTION WAS SUPPORTED WITH 4' RESIN BOLTS AND SUPPLEMENTED WITH 6 10' CABLE BOLTS. NO INJURIES RESULTED FROM THIS FALL. FALL WAS APPRX 40' X 26'X 10' HIGH.

Struck against a moving object

EE was backing bridge system out of a cut, while doing so the outby edge of operator's canopy caught a roof bolt head, throwing EE against arm rest and panic bar, causing injury.

Over-exertion in lifting objects

EE REPORTED ON 12/28/04 THAT HE WAS INJURED ON 12/14/04. WHILE LIFTING A MINE DOOR AT THE PORTAL, HE EXPERIENCED PAIN IN LOWER LEFT ABDOMEN. EE HAS NOT MISSED ANY WORK. NOTE: HAD SURGERY 1/10/05 & RETURNED 2/14/05, MISSED 25 DAYS.

EMPLOYEE STATED THAT HE JARRED HIS BACK WHILE OPERATING A SCOOP. FELT PAIN IN LOWER BACK. DID NOT START MISSING WORK UNTIL 11/23/04.

EE notified us that he was going to the doctor for an incident that occurred on 11-7-04, ee stated he felt pain in his lower back while working on miner ripper head. EE has had ongoing back pain. Did not start missing work until 11-29-04.

EMPLOYEE WAS STRUCK BY A 3"" WATER LINE WHEN A COUPLING FAILED UNDER PRESSURE. THE COUPLING HAD BEEN LEAKING. WATER WAS TURNED OFF, COUPLING WAS TAKEN APART, CHECKED AND TIGHTENED. THE WATER WAS TURNED ON SLOWLY. COUPLING HELD FOR THREE MINUTES AND THEN BLEW APART.

EMPLOYEE WAS STRUCK BY A PIECE OF DRAW ROCK WHILE INSTALLING A SCRAPPER AT A HEAD DRIVE. THE ROCK STRUCK HIM ON THE HEAD AND BACK CAUSING A COMPRESSION OF T7 DISC.

EMPLOYEE EXPERIENCED PAIN IN LOWER ABDOMEN WHILE LOADING A BOX OF RESIN ONTO A ROOF BOLTER. EMPLOYEE WENT TO DOCTOR AND WAS DIAGNOSED WITH A HERNIA.

Employee was drilling a roof bolt hole, while doing so, he lost his balance and caught his hand on rotating chuck of drill, resulting in twisting injury to ee's right forearm.

Mechanic was standing on diagonal brace of d10 Dozer. The ee's foot slipped off of bace and he fell between brace and main frame.

EE STATED HE WAS DRILLING HOLES FOR ANCHOR BOLTS IN MINE FLOOR FOR BELT HEAD DRIVE USING HYDRAULIC SHORDER DRILL WHEN DRILL BIT GOT HUM=NG IN CRACK CAUSING DRILL TO SURGE/TWISTING STRIKING HIM ON HIS LEFT FOREARM. EE SUSTAINED FRACTURE TO THE UINA BONE LEFT ARM.

EE WAS RECOVERING A 4"" WATER LINE AT TURTLE PORTAL USING A BATTERY POWERED SCOOP, HAD WATER LINE TIED TO SCOOP BUCKET WITH ONE JOINT OF PIPE EXTENSION BACK OVER SCOOP ON OFFSIDE WHEN PIPE GOT HUNG ON CANOPY POST AT PORTAL CAUSING PIPE TO SWING AROUND STRIKING EE IN THE FACE. EE SUSTAINED A FRACTURED NOSE & LACERATION ABOVE HIS RIGHT EYE.

OPERATOR DECK OF SHUTTLE CAR CAUGHT THE SIDE OF A PIECE OF DRAW ROCK WHICH WAS IN AN INTERSECTION ANS SHOVED IT AROUND ONTO MINER HELPERS LEG CAUSING INJURY. THE DRAW ROCK HAD BEEN CLEANED OFF THE TOP OF THE CONTINUOUS MINER.

WHILE TRAVELING FROM TOOL CAR TO THE CONTINUOUS MINER HIS RIGHT FOOT GOT CAUGHT ON A PIECE OF 4' X 3' BELT LAYING ON THE MINE FLOOR CAUSING HIM TO TRIP AND FALL. HE TWISTED HIS RIGHT KNEE.

Ee was engaged in removing a deck from the coarse coal screen. The deck became wedged. When ee freed the deck with a slate bar the deck struck his foot.

WHILE WORKING ON A PICK-UP THAT HAD A FLAT, EE PICKED UP A ROCK TO SCOTCH THE WHEEL, HE PICKED UP THE ROCK & TURNED HIS BODY AT THE WAIST & FELT A SHARP PAIN IN HIS BACK.
