Accident type, without injuries

A pillar fall occurred on the MMU 003-0 in the #11 crosscut in the #3 pillar, catching the continuous miner, size of fall undetermined. No injuries, ventilation was not interrupted.

An unintentional roof fall occurred at 27 break along the #5 underground belt conveyor in the #3 entry, size 25'L x 4 1/2'H x 18'W. Supported with 4' fully grouted bolts. No injuries, no equipment caught and ventilation was not interrupted. Fall will not be cleaned up.

Accident type, without injuries

An unintentional roof fall occurred at the #14 brk, 3 West #2 belt in the #3 entry. Size of fall was estimated to be (5'H x 18'-20'W x 70'L). Fall will not be cleaned up. No injuries occurred, no equipment caught, ventilation was not interrupted. Area had been bolted with 6' torque tension bolts.

An unintentional roof fall occurred on the 001 section in the last open break in the #2 entry. Size of fall was estimated to be 60' L x 18'-20' W x 8'-12' H. Fall will not be cleaned up. Section has been abandoned, equipment being removed from area. No injuries occurred, no equipment caught, ventilation was not interrupted.

An unintentional roof fall occurred at 48 break along the #5 underground belt conveyor in the #7 entry, size 18'Lx4'Hx20'W. Was supported with 4' fully grouted bolts. No injuries resulted from this fall. Roof fall will be cleaned up. Roof fall was found during the weekly examination.

An unintentional roof fall occurred at 24 break along the #4 underground belt conveyor in the #5 entry, size 60' Long X 7' high X 20' wide was supported with 4' fully grouted bolts with 10' cable bolts installed at intersections. No injuries, no equipment caught & ventilation was not interrupted. Fall will not be cleaned up. Roof fall was found during weekly examination.

A fall of supported roof has occurred outby the active sections along the #3 conveyor belt in the #5 entry 1 break inby spad station #1083. The fall measured approximately 20' wide x 8' high x 40' long. No injuries resulted from this fall.

An event occurred (ignition) in behind the old #13 belt seals, one of the 7 seals was dislodged, eliminating the separation between the active and sealed area of the mine. Cause of ignition unknown.

Struck by falling object

Employee was in a kneeling position organizing roof bolting supplies on the Fletcher roof bolter when a rock fell from between bolts striking him on the ankle of his right foot. Rock measured 36"" long x 26"" wide x 2"" thick.

Employee was cleaning up spilled coal at take up and stepped on some loose coal & twisted his ankle.

Struck by falling object

Went to stopping and bent over to pick up a bucket of plaster. When employee raised back up, a piece of draw rock 12"" x 6"" x 1/4"" thick fell on his left forearm.

Caught in, under or between a moving and a stationary object

Employee was removing belt structure when his index finger on his left hand got caught between two pieces of belt structure. Employee sustained a fracture/laceration to the distal joint of the index finger.

EE was swinging lare componet (777A Brake unit) with service truck boom. He was watching the top of the boom to check clearance while swinging boom. As a result he was trapped between compartment and service truck.

Over-exertion (Not Elsewhere Classified)

Injured was carrying two cans of hydraulic oil from #3 entry to the #4 entry, when he felt pain in his lower back. Employee did not go to hospital and worked his next scheduled shift. Employee started missing work on 4/4/2006.

Struck by falling object

EE was struck on the back by a rock brow that broke out from around roof bolts (10-12"" thick, 18-48""wide and 90-96""long). EE sustained fracures to left ribs, lower back and neck. Accident occurred on the MMU 003-0 section.

Over-exertion (Not Elsewhere Classified)

EE was loading oxygen & acetylene tanks onto personnel carrier when he felt pain in his lower abdomen. EE did not start missing work until 2/2/06.

Struck by flying object

EE was observing a 6"" water line that he had helped relocate from the off side of the 001-0 section belt to the track side of the belt. When pipe was pressurized, a roustabout coupling came loose allowing pipe to swing around and strike him on his forehead. He sustained lacerations above left eye and right temple, requiring sutures.

Over-exertion in lifting objects

AS EMPLOYEE ATTEMPTED TO LIFT A 5 GALLON PAIL OF HYDRAULIC FLUID TO REFILL MACHINE RESERVOIR. HE EXPERIENCED LOWER BACK PAIN THAT QUICKLY SUBSIDED.

Struck by falling object

Employee stated while loading supplies in scoop bucket a piece of rock fell from top striking him & knocking him to mine floor, trapping him underneath the rock. He was retrieved from under rock, transported to surface, transported to Holston Valley Medical Ctr, Kingsport, TN. He was kept overnight for observation, released (02/06/06).
