Accident type, without injuries

An inundation of gas occurred on MMU 001-0 Section. While advancing Panel #16 cutting Gob-I on across from #6 right crosscut at #27 break. East Mains sealed area was cut into at #27 break on panel #16. This was located approx 100' from #6 entry survey station #3325, w/last 25' unsupported. No injuries reported, temp vent.controls constructed & seal built according to plan.

Accident type, without injuries

An unintentional roof fall occurred #3 North Belt and Track entry at spad #1902. Size of fall was estimated to be 150'L x 20-22'W x 4-7'H. The area had been supported with 5' fully grouted resin bolts with supplemental support consisting of 9' cable bolts. No injuries, ventilation was not interrupted, belt and track were caught under fall. Fall has been cleaned up.

An unintentional rock fall was discovered in the #1 entry beginning at spad #2129. The area had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall will be cleaned up.

A fall of supported roof occurred at 87 crosscut extending to 88 crosscut in #3 entry, #3 belt. The fall measured 80' long x 20' wide x 8' high.

A FALL OF SUPPORTED ROOF OCCURRED ON THE 002 SECTION 1 BREAK INBY 9A HEAD DRIVE IN #4 ENTRY. THE FALL MEASURED 30' W X 30' L X 10' H. NO INJURIES RESULTED FROM THIS FALL.

A FALL OF SUPPORTED ROOF OCCURRED AT 105 BREAK ON THE WEST MAINS, BETWEEN #3 & #4 ENTRIES. THE FALL MEASURED 8' - 10' H X 20' W X 80' L.

A fall of supported roof was found in an outby area on west mains #1 entry between break 88 and 89. the fall measured 20'wx85'lx10'h. no injuries resulted from this fall.

AN UNINTENTIONAL FOOR FALL OCCURRED IN THE #4 ENTRY (BELT) INBY SPAD #9398 3 SOUTH MAINS. SIZE ESTIMATED TO BE 160' L X 18-20' H. THE AREA HAD BEEN SUPPORTED WITH 5' TORQUE TENSION BOLTS WITH SUPPLEMENTAL SUPPORT CONSISTING 10' AND 16' ROPE BOLTS. NO INJURIES, BELT LINE CAUGHT, VENTILATION WAS NOT INTERRUPTED. FALL WILL BE CLEANED UP.

A fall of supported roof occurred on the 002 section at #1 break #7 entry inby spad 9746. The fall measured 35' l by 18' w by 8-10' h. No injuries resulted from this fall.

A FALL OF UNSUPPORTED ROOF OCCURRED ON THE 002 SECTION AT # 3 CROSSCUT, # 4 ENTRY AT SPAD STATION 9481. THE FALL MEASURED 20' L X 20' W X APPRX 7' HIGH. NO INJURIES RESULTED.

A fall of supported roof was found during weekly exam, in an outby area. The fall is located at 67 break on west mains no.1 entry, 50' inby spad station 7345. The fall measured 25' l by 20' w by 5' h.

An unintentional roof fall was discovered in the #2 entry one break in by spad #58. Size estimated to be 75' l by 18-20' w by 8-10' h. The area had been supported with 6 foot torque tension bolts with supplemental support consisting of 9 ft cable bolts. No injuries, no equipment caught, ventilation was not interrupted.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE #1 ENTRY ON 2 LEFT OFF 2 NORTH MAINS 1 BRK INBY SPAD #2646. SIZE 60' LONG X 4' HIGH X 18-20' WIDE. WAS SUPPORTED WITH 4' FULLY-GROUTED RESIN BOLTS. NO INJURIES, NO EQUIPMENT CAUGHT, VENTILATION WAS NOT INTERRUPTED. FALL WILL NOT BE CLEANED UP.

AN INTENTIONAL ROOF FALL RODE OUTBY THRU BREAKER POST INTO ACTIVE RETREAT MINING SECTION. THE OUTBY EDGE OF FALL CAUGHT THE FRONT MBC OF A CONTINUOUS HAULAGE UNIT. ALL PERSONNEL WERE PULLED OUTBY BEFORE FALL OCCURRED. NO INJURIES RESULTED FROM THE FALL. APPROXIMATE SIZE OF FALL IS 100'L X 10'-20'W X 12'H. LOCATION IS ON S. MAINS #11 X-CUT #3 ENTRY.

A fall of supported roof has occurred in by spad # 2589, 001 section. Number 15 crosscut, #2 entry. No injuries resulted from this fall. The fall measured 45' l by 15-20' w by 10' h.

An unintentional roof fall was discovered on the 001-0 (West) Section in the #2 entry, spad #2576. Size: 50'L x 4-9""H x 18-20""W. Had been supported with 4' fully grouted resin bolts with supplemental support consisting of 9' rope bolts. No injuries, no equipment caught, ventilation was not disturbed. Fall will not be cleaned up.

A FALL OF SUPPORTED ROOF WAS FOUND ON THE 002 SECTION NO. 9A BELT AT #2 CROSSCUT INTERSECTION OF NO. 7 ENTRY. THE FALL MEASURED 20'W X 20'L X 6'H. NO INJURIES RESULTED FROM THE FALL.

An unintentional roof fall occurred on the 001-0 (west) section in the #4 entry (belt entry) spad #2613. Size: 4'-5'H x 60'L x 18'-20'W; had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall has been cleaned up.

A fall of supported roof occurred in no. 4 entry between crosscut #7 and 8 on south mains, parallel entries on 001 section. The fall measured 18' wide by 70' l by app 6' high. No injuries resulted from the fall.

A fall of supported roof occurred on No. 5 belt (south mains) No. 4 entry (belt enrtry) at No. 14 crosscut. The fall measured approximately 35' long x 20' wide x 12' high. No injuries resulted from this fall.

Ee sustained injury to his left big te when an airlock door came in the deck of personnel carrier he was riding in EE sustained crushing injury to his left big toe. EE big toe was amputated to the first joint.

Contact with electrical current

Employee stated he was attempting to make corrections in the Pemco 1500 KVA section power box. While he was working in the section power box he came in contact with an electrical current, resulting in an electrical burn to his right hand palm and elbow.

Struck against stationary object

Shoveling coal, he hit his hand on structure, fracturing his left hand.

Injured was operating roof bolter installing roof bolts when drill steel hit him on his left forearm. resulting in a fracture to forearm.

Struck by falling object

Employee was operating miner on the 002-0 south section, was taking a lift cut in the #1 gob eye when a rib roll struck employee on the back of his left leg knocking him down. Employee sustained fracture to the tibia bone just above the ankle.

Caught in, under or between a moving and a stationary object

Employee was loading supplies onto motor, operator told him he was moving. Injured employee did not realize how close he was to motor. The motor caught the tip of his left boot and rolled it over against the ground, causing an injury.

Over-exertion in lifting objects

EMPLOYEE FELT PAIN IN LOWER BACK AFTER PICKING UP A PIECE OF BELT STRUCTURE IN SUPPLY YARD. EE INFORMED ME ON 4/7/05 THAT HE STARTED MISSING WORK ON 4/1/05.

Struck against stationary object

EE WAS HELPING INSTALL BELT IN #1 BELT LINE. A DIESEL MOTOR WAS BEING USED TO PULL THE LOOPED BELT OFF A FLAT CAR. THE BELT SLIPPED OFF THE FLAT CAR, STRUCK EE, SPUN EE AROUND AND INTO THE BELT STRUCTURE CAUSING INJURY. EE WAS POSITIONED BEHIND THE FLAT CAR AT THE TIME OF THE INCIDENT

Over-exertion in lifting objects

Employee stated while removing a bag of rock dust (40 lb) from scoop, he felt pain in his lower back. Employee started missing work on 3-14-05. EE was diagnosed as having low back strain.

Flash burns (welding)

Injured employee was supervising and assisting co-worker who was welding a cover to the end of a steel pipe. While in close proximity to welding arc, injured employee unknowingly exposed his eyes to excess radiation, thereby incurring minor eye burn.

Struck against a moving object

OPERATING BULLDOZER. DOZER ROLLED OVER WHEN EE LOST CONTROL WHILE RECLAIMING STEEP SLOPE. THE DOZER GOT ONTO A TREE TRUNK WHICH CAUSED IT TO SLIDE. EE WAS WEARING HIS SEAT BELT; HOWEVER, HE STRUCK A HARD SURFACE INSIDE CAB, CAUSING AN INJURY (LACERATIONS TO FACE).

Employee was helping make belt move, belt was being taken out, in the process of removing top belt rail ee's index finger on right hand got caught between belt rail and mine roof. Employee sustained laceration/ contusion to index finger (rt) requiring sutures.

Caught in, under or between a moving and a stationary object

Employee pinched fingertip while riding in scoop bucket, possibly by ejector, employee was not sure. The ejector was chained and had two inches of clearance. Employee was wearing metacarpal gloves.

Over-exertion (Not Elsewhere Classified)

Repretitive motion over long period of time. Ee went to dr on 4-4-05. reported this to me on 5-2-05.
