An unintentional roof fall occurred in the #5 entry at spad #731. Size of fall was estimated to be 22'W x 22'L x 5-6'H. The area had been supported with 4' fully grouted resin bolts with additional support consisting of 2 cribbs. No injuries, ventilation was not interrupted, no equipment caught. Fall will not be cleaned up.

An unintentional roof fall was discovered in the #9 entry at #28 break (Intake). Size of fall was estimated to be 50'L x 20'W x 5'H. The area had been supported with 4' resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall will not be cleaned up.

An unintentional roof fall was discovered at spad #962 in the cross cut between #6 & #7 entries (returns). Size of fall was est to be 35' L X 18'-20'W X 6' H. An unintentional roof fall was discovered 30 feet outby spad #985 in the #6 entry (return). Size of fall was estimated to be (50'L x 20'W x 4'-5'H.) Area had been supported with 4' fully grouted resin bolts with supplemental support consisting of 10' cable bolts in the intersections. No injuries, no equipment caught, ventilation not interrupted. Fall will not be cleaned up.

An unintentional roof fall was discovered between #2 & #3 entry at spad #785 at break #28. Size of fall was estimated to be 5'H X 18'-20'W X 55'L. Fall will not be cleaned up. No injuries occurred, no equipment caught and ventilation was not interrupted. Area had been bolted with 4' fully grouted resin bolts. Fall will not be cleaned up.

An unintentional roof fall occurred in the #4 entry at the #67 crosscut on the #3 belt. The fall measured approximately 20' wide x 6' high x 40' long. Area was supported by 4' fully grouted bolts. No injuries, no equipment caught & ventilation was not interrupted.

Struck by falling object

EE was operating continuous miner in the #4 heading was cutting right lift. Approx. 25' when top broke starting at the face falling outby into intersection. Positioned on the right side of the intersection miner from kneeling position located between the 3 & 4th row of bolts outby the face. Head and body trauma resulting in death

Attempting to raise belt head with lifting jack to reset crib, his hand slipped and jack bar came out and hit him, resulting in a cut to his chin requiring sutures.

Struck by falling object

Injured was kneeling on his right knee at intersection, 4 rows back from face operating continuous miner, taking 1st cut of of 1 left gob eye on 004 Section. A rock fell out from between bolts striking him on his back & right hip, resulting in fracture to rightt hip. Rock measured approx. 71"" long x 36"" wide x 1/4"" to 5"" thick & mining height was approx. 82"".

Caught in, under or between (Not Elsewhere Classified)

EE was positioned between #3 mobile bridge and the low-low structure, right foot on the low-low belt, left foot on mine floor. EE was attempting to bump conveyor chain, in this process, his arm or shoulder engaged the right tram causing the bridge to move forward, catching his left leg between the mobile bridge and low-low structure. Fractured his fibula and cut his calf (sutures).

Caught in, under or between a moving and a stationary object

Employee was tramming scoop through block curtain battery end first when curtain came in contact with canopy. Employee said his vision was blocked. He raised curtain up with his left hand inching scoop thru curtain when his left hand got caught between rib and canopy. Employee sustained fractures to his index finger and thumb.

Struck by falling object

Employee was riding in an open mantrip in the 3 west #4 belt entry when a rib rolled as the mantrip passed this area striking the employee on his left shoulder and left rib cage. Rib roll was approximately 126"" L x 1""-4"" thick x 46"" H, height in the area was 86"". Employee sustained enclosed concussion, collapsed left lung, fx left ribs.
