A Rock Fall was discovered at 71 break on the track entry the fall was 20' wide x 30' long x 6' to 7' thick the area had been supported with 4' fully grouted resin bolts, no injuries, travel was blocked. The fall will be cleaned up.
An unintentional roof fall was discovered in front of the #1 seal of the 14 seal area. Size of the fall was estimated to be 20' L. X 20' W. X 6' Thick. No injuries, ventilation was not interrupted and no equipment was involved. The fall will be cleaned up.

The driver was hauling coal to the plant from spruce creek was starting down a hill the truck got out of gear and the engine quit on the truck he could not get it back into gear he went down the hill and turned over.
Bitten by stray dog on mine property, bite to right forearm. Employee was getting lunch out of vehicle when dog approached.
