An unintentional roof fall was discovered between break 33 and 34 in the #2 entry the fall was 30' land X 19' wide X 4'6"" high. This fall will not be cleaned up and is timbered or cribbed on all sides and dangered off. The fall does not effect travel or ventilation.

A Rock Fall was discovered at 71 break on the track entry the fall was 20' wide x 30' long x 6' to 7' thick the area had been supported with 4' fully grouted resin bolts, no injuries, travel was blocked. The fall will be cleaned up.

An unintentional roof fall was discovered in front of the #1 seal of the 14 seal area. Size of the fall was estimated to be 20' L. X 20' W. X 6' Thick. No injuries, ventilation was not interrupted and no equipment was involved. The fall will be cleaned up.

An unintentional roof fall was discovered at 18 break along the # 6 belt in the reurn in # 3 and # 4 Intersections. Size of the fall was 4' to 5' thick the area is dangered off. No injuries, ventilation was not disturbed and no equipment was involved. The fall will not be cleaned up at this time.

Rock fall was discovered in the intersection at 50 break in the #3 entry along the #1a belt. The fall was 5' high x 20' wide x 30' long. No injuries and no ventilation controls or equipment was effected. This fall will be not be cleaned up and the area has been dangered off.

An unintentional roof occurred at break 104 in the #4 entry (belt). Size of fall was estimated to be 12'L x 7'W x 3'-31/2'Thick. No injuries, ventilation was not interrupted, belt line was caught. Fall will be cleaned up. The roof had been supported with 6' fully grouted resin bolts

The driver was hauling coal to the plant from spruce creek was starting down a hill the truck got out of gear and the engine quit on the truck he could not get it back into gear he went down the hill and turned over.

Bitten by stray dog on mine property, bite to right forearm. Employee was getting lunch out of vehicle when dog approached.
