Struck by falling object

While installing a roof bolt, a rock was dislodged and struck ee on the shoulder.

Employee stated that while getting out of the truck, his foot got caught causing him to fall, he stated that his left leg and back hurts.

Employee was removing a fuel pump from truck, when he attempted to get off the truck he lost his footing and fell approximately 6'.
