EE stated that while cleaning up around No 4 belt head he slipped on a rock and fell. When he got up he could not put much weight on his foot.

EE was working to construct an overcast. He had stood up a metal leg and while attempting to place a bolt in the leg it fell over and struck him on his lower leg.

EE was bolting top when a piece of rock fell on EE's hand.

EE was shoveling the belt when he tripped and was struck above the eye by a shovel held by a co-worker. EE alleges knee injury. The accident is being contested. (See Attachment)
