Accident type, without injuries

A roof fall measuring approx. 20' x 20' x 6' occurred on 7 right section #3 entry near spad no. 2595.

Caught in, under or between a moving and a stationary object

Employee was observing the mechanic working on a shuttle car. It was revealed during the accident investigation, that the mechanic pressed the tram pedal, causing the shuttle car to inadvertently move. The shuttle car crushed the employee between the shuttle car and the rib.

Flash burns (electric)

Employee was working on a 480 volt receptacle. When he opened the dust cover, the receptacle flashed ""arched"" causing burns to the back of employee's hands.

Struck by powered moving object

Employee was checking the operational controls on the 21-SC shuttle car, when the tram pedal was pressed. The shuttle car inadvertently moved, causing the shuttle car to strike the employee.

Caught in, under or between a moving and a stationary object

Opened Air Lock Doors and air caught doors and blew back on the left arm of EE.

Struck by... (Not Elsewhere Classified)

EE was bolting top in #7 heading on 1st left section, finished his row of bolts and was getting ready to advance pinner, when rock broke loose in between bolts and struck him on the head and shoulder.
