Accident type, without injuries

A roof fall above anchorage was found in the Mains Primary Intake Escapeway approximately 5,500 feet outby Mains 1st Left Section. All approaches to fall area was cribbed off. Life line was rerouted and all employees instructed on new escapeway entry. Fall will not be cleaned up.

Accident type, without injuries

A roof fall occured on the 1st right section in the #3 heading, 1 break inby spad #6029. The fall was approx. 8' X 18' X 22. No persons injured and no equipment was involved.

Over-exertion (Not Elsewhere Classified)

Employee stated that while walking across section, he stepped in a hole and twisted his knee and back.

Flash burns (electric)

Employee stated that while unplugging the scoop batteries, a wire grounded inside the plug. This caused an arc from the plug.

Struck by... (Not Elsewhere Classified)

On 3/10/11 the employee reported that while pulling belt with a scoop, the scoop slid over and the battery end of the scoop struck him on the leg. Employee did not miss work until 3/14/11.

Struck against a moving object

While operating the rubbered tired mantrip the EE struck the rib and jarred his low back.

Fall from machine

EE was cleaning the miner, while getting off the miner the EE slipped and fell causing him to injure his lower back. EE did not miss work until 3/10/11.

Caught in, under or between a moving and a stationary object

Employee was helping on roof bolter when the roof bolter operator allowed the drill head to come down on his foot.

Employee was pulling lid back on a oil can when his hand slipped and struck sharp edge.
