A roof fall above anchorage was discovered in the Mains Intake Air Course at Survey Station 266 approximately 6000 feet from the Mains Section. The roof fall measured approximately 20' wide by 20' long by 5' high. All approaches have been dangered off and cribbed.

Employee was removing a canopy from a scoop bucket and later felt a pain in his abdomen.

EE was setting in the shuttle car deck when a piece of rock measuring 16"" X 12"" X 2"" thick rolled from the rib and struck him on the ankle.
