Accident type, without injuries

An unplanned roof fall above anchorage was discovered during the Preshift Exam at Survey Station 7893, approximately 1200' outby the 2nd Left working section. The fall measured approximately 20'L x 20'W x 9'H and has been dangered off from all approaches.

Struck against a moving object

Employee was operating a shuttle car when he hit a pot-hole in roadway causing the employee to hit his head against the canopy.

Employee was turning a mantrip around when the front tires contacted a ledge causing the steering wheel to spin and the steering knob struck the employee's right hand.

Struck against stationary object

Employee was riding in mantrip to section when he stated that he hit his hardhat on a roofbolt.

Caught in, under or between a moving and a stationary object

Employee was installing a roof bolt with a bearing plate when he attempted to straighten the plate and pinned his middle finger on his right hand to the mine roof.
