A ROOF FALL WAS DISCOVERED IN THE 2ND MAIN NORTH #3, IN #1 ENTRY, CROSSCUT 33 TO 34. THE FALL MEASURES APPROX. 18'WIDE X 9'HIGH X 60'LONG. PRIMARY SUPPORT IN THIS AREA IS 5' FULLY GROUTED RESI N BOLTS.
EMPLOYEE WAS GOING TO WORK ON HAWKEYE AT THE SECTION TAILPIECE. HE SHUT BELT OFF AT TAILPIECE. BEFORE THE TAILPIECE QUIT ROTATING, HE REACHED IN TO PERFORM WORK. HE CAME INTO CONTACT WITH THE TAILPIECE WITH HIS LEFT MIDDLE FINGER. THE LEFT MIDDLE FINGER SUSTAINED A FRACTURE.
As employee placed remote box on top of continuous miner after tramming, a piece of rock fell from between rows of bolts. The rock struck his right hand severely lacerating some fingers.
