A roof fall occurred in the 2 West 1 Mains, # 4 entry, at crosscut 64. The fall measures approximately 6' thick, 50' long, and 18' wide. The area is supported with 5' fully grouted resin roof bolts.

Employee was a passenger in the mantrip when it ran into a rib while sliding thru a curve. Passenger hit his head on the front plate of mantrip. Per procedure employee was life flighted to hospital. No injury was reported.

Employee was closing the manually operated equipment doors located in the 3 North 1 Mains. Upon closing the first door, the second door closed on his left forearm. The employee sustained a fractured left forearm.
Employee stepped out of coal hauler on # 1 unit, # 8 entry, at crosscut # 8 and stepped onto a rock and twisted left knee. Employee continued to work until surgical intervention was required on January 14, 2010. This incident became reportable on January 14, 2010, and is a charge back to November 05, 2009.
EE trammed minitrac in front of feeder and into crosscut, cal hauler padded under curtain while ringing bell. Minitrac & coal hauler contacted on another neither piece of equipment was damaged and pie pans did not fall off of forks.
EE was roof bolting on #3 unit,#2 rt. face. EE pryed draw rock with channel locks and rock fell mashing his left thumb on top of canopy. The EE sustained a simple fracture of the left thumb.
EE was rock dusting when hose slipped out of his hand and dust blew into his left eye. Management was made aware that days missed from work were attributed to incident on 10/09/09.
EE was operating the continuous miner under supported roof on #4 unit, #4 entry crosscut 58 to 59 when a piece of roof rock dislodged from between the row of roof bolts and the right coal rib. The ee sustained simple non displaced fracture of the right transverse process.
Employee was pulling pin to unhook duster from getman tractor when right index finger was inadvertersly placed in pinch point and mashed.
The employee was trouble shooting a hydraulic problem on a piece of equipment. The employee stood up from a stooped position and felt pain in the right knee. The employee continued to work until surgical intervention was required on November 12, 2009, at this point the incident became an MSHA reportable ""accident"".
Employee was walking down steps in prep plant when he caught back of boot on step and decended 4 steps to the concrete floor. Employee sustained a simple undisplaced fracture of a bone in the left foot.
Employee was building block stopping when block fell over striking left ankle. Employee sustained a sprained left ankle.
Employee was adding sectional roof bolt steel into hold when steel did not line up and snapped back, steel struck left hand. He sustained a simple fracture of the left index finger.
Employee was driving in diesel mantrip when she drove through bulk dust that had been placed on the roadway. The rockdust flew up & obscured vision. She hit the coal pillar with the vehicle. She sustained a cracked orbital socket on the right side.
EE was walking underground when eh claims left ankle just ""gave out"". EE missed workd on 4/9/09.
Employee was cutting #7 right at 36 crosscut when rock fell in unsupported roof area and cantalevered into supported roof area striking right ankle. Employee sustained a simple fracture of the right ankle.
Employee was cutting ventilation curtain with knife when he cut left wrist lenghth wise 3/4"" long. Employee continued to work but missed work due to this incision on 3/9/09.
Employee place 6x6 timber under stab jack of continous miner and inadvertently placed right hand in between timber and rear, bumper. As machine jack was moved hand was pinched. He sustained simple fracture of middle finger and carpal bone in hand.
EE was carrying belt rollers through the crosscut when he tripped on metal band canding falling onto left shoulder. EE continued to work until 09/10/09 when he had surgery on shoulder.
Employee was attempting to cap off concrete block brattice when brattice fell over. Concrete block struck employee's lower left leg. Employee sustained a simple fracture to the left lower leg.
Employee was getting roof bolt off of machine when cable came loose from tie wire on roof and cable struck employee. He has soreness in shoulder blade area.
Employee was walking to equipment when bolter cable came untied from roof and struck employee on the side of his head. Employee sustained a concussion.
THE EMPLOYEE WAS A PASSENGER IN A MANTRIP WHICH CAME IN CONTACT WITH THE COAL RIB. THE EMPLOYEE'S HEAD STRUCK THE INTERIOR FRONT PLATE OF THE MANTRIP. THE EMPLOYEE WAS TRANSPORTED TO THE DEACONNES HOSPITAL VIA LIFE FLIGHT. HE REMAINS IN THE HOSPITAL AT THIS TIME.
EE was attaching rockduster to getman tractor, when pin mashed left little finger, requiring sutures.
Employee was backing up mantrip when he hit parked scoop. The employee struck the left brow on the canopy post, resulting in a laceration that required 3 sutures.
2 maintenance employees were attempting to load a 1/4"" thick plate metal onto a maintenance mantrip in order to transport to their work site, when the edge of the 1/4"" thick metal plate slid and caught the mechanics left middle finger, as it was falling to the ground.
Employee was helping load bundle of bolts onto roof bolter. He placed board between bolts and scoop in order to move bolt bundle forward. The tip of the left index finger was placed in pinch point between the board and the bolts. Tip of the finger was lacerated requiring sutures.
