An unintentional roof fall occurred on 2 South 2, Crosscut 16 between Entries 5 and 6. The roof fall measured approximately 40' in length, 18' in width, and 8' in thickness. A 103(k) Order (8444691) was issued August 20, 2014, by MSHA Inspector.

Employee was traveling in a mantrip when he hit a block causing him to bounce around in mantrip. Employee sustained a fracture to his back and will remain off work for an uncertain period of time.

Absorption of radiations, caustics, toxic and noxious substances

Employee was working in water and grout from the drill hole. Employee sustained 3rd degree burns and remains off work for an uncertain period of time.

Fall to the walkway or working surface

Employee was walking when he tripped and fell and felt a snap. Employee went to hospital with a broken leg and will remain off work for an uncertain period of time.

Caught in, under or between a moving and a stationary object

Employee was installing split steel and didn't know his glove was caught. Employee went to Good Samaritan Hospital sustaining a fracture to his finger which requires surgery and will be off work for an uncertain period of time.

Employee was directing his co-workers when a band was cut holding the prop setters together and the props rolled onto employees foot. Employee sustained a fractured foot and returned to work on restricted duty the following work day.

Employee was using a utility knife and slipped cutting into his leg. Employee immediately went to Good Samaritan Hospital receiving sutures.

Employee was carrying a roller when he hit his finger against a timber requiring him to get sutures.

Employee was breaking a steel band when it sprung back cutting his arm. Employee required sutures and returned to work the following work day.

Employee was reaching to pick something up when he cut his hand on slag. Employee required sutures and returned to work the following work day.

Employee was working on a heater when he cut his finger on a piece of metal. Laceration required stitches and employee returned to work the following work day.
