Fall from machine

While climbing down the machine, employee stepped on shoe string and fell to the ground.

Over-exertion (Not Elsewhere Classified)

Employee was removing a rock from under a 16"" hdpe pipeline with a shovel. Ground had been softened by recent rains. While applying force on the shovel, it rotated, twisting the employee's leg. The twisting force caused the employees kneecap to rotate approximately 45 degrees coming to rest on the ouside of his left leg.

Fall to the walkway or working surface

Employee slipped and fell while cleaning up spilled hydraulic fluid.

Absorption of radiations, caustics, toxic and noxious substances

Mechanics helper inhaled the fumes from a can of brake cleaner as the mechanic was spraying a part on the excavator.
