Caught in, under or between a moving and a stationary object

Employees were placing 24"" HDPE pipe, when the pipe suddenly shifted and rolled on top of one of the workers. He was treated and stabilized at the scene. He was subsequently transported to a hospital, where he later passed away.

Over-exertion in lifting objects

Employee bent over to pick up pvc pipe and sprinkler assemblies. He stood up and felt back pain.

Struck by falling object

Employee was working under wheel well of haul truck when he was struck by a falling piece of mud.

Struck against stationary object

Employee was washing 429 van when the hose he was using got stuck. When the employee flipped the hose to get it loose, he struck the van with his elbow causing a contusion.

Struck by falling object

EE was jackhammering concrete at the tankhouse when a piece he was working on broke off. The jackhammer fell on the EE's foot. The x-rays showed that the EE's toe next to his big toe was fractured. The EE was restricted to 3 days of light duty.

Over-exertion in pulling or pushing objects

Employee was working at the lower quickfill. He was removing bad order cable. He bunched up the cable with his green strap and pulled the slack out of the strap. When he finished, he felt pain in his right hand

Employee was moving copper bars out of the way at cell 18. The last bar slipped out of his hand, smashing his right index finger between the copper bar and the bus bar.
