Fall from machine

Employee slipped while exiting the Grader on which he was working and slid down 3-4 feet causing him minor abrasions to both elbows and stiffness in middle finger. Emp was x-rayed, cleared for duty. He rec'd surgery consult on right shoulder & assigned no work status 3-15 while on paid investigatory leave for a separate matter

Struck against stationary object

Employee was spraying down harvested cathodes with a 1"" water hose. When he stepped on the hose, he twisted his knee.

Fall from machine

Speed/Haste - Employee failed to maintain three points of contact as instructed. Employee failed to recognize foot was caught in step when dismounting equipment. Employee suffered bruise to torso and strain/sprain in his shoulder. Employee has made full recovery and returned to work.

Caught in, under or between a moving and a stationary object

Employee caught middle finger of right hand between starter sheet blank bar and conveyance chain causing laceration.

Over-exertion in pulling or pushing objects

Employee was pull starting a diesel sump pump several times during the shift causing muscle soreness

Over-exertion in wielding or throwing objects

While swinging a hammer right arm began to hurt.
