Holding a bolt with his fingers instead of a wrench and tried to spin it free with an air wrench cutting hand.

Climbing down steps of loader and slipped falling to ground and fracturing his ankle.

Pulling belt with a fork lift, connection came loose and clamp hit windshield which shattered putting small glass shards into his eyes.
