Struck against stationary object

Stepping from cab of dozer onto step. During work a rock had become stuck in the step which caused him to roll his ankle and foot.

Fall onto or against objects

Tripped over boards which had tipped over into walkway causing him to stumble and hit ribs on a metal table. Did not seek medical attention for several days. Condition did not improve.

Caught in, under or between a moving and a stationary object

Cleaning material stuck in the bed of a dump truck. EE lifted the bed so the bed would stay open, connected the chains as an extra precaution and started cleaning the material. During the operation she bumped the chains causing the tailgate to dislodge and swing shut smashing fingers on both of her hands.

Struck against a moving object

running a dozer and while backing to make another push ran over a large rock jarring his back and neck

Struck against stationary object

Changing a cutting edge and bolt was stuck. He used his hand to hold bolt instead of a wrench and when it spun it cut through glove lacerating his hand.

Working a pin into leg of crusher with hammer, pin flipped back and hit employee in eye.
