Struck against stationary object

Checking screens on a wet shaker and stepped over a wash bay onto a j bolt which penetrated into shoe and slightly into foot.

Struck by falling object

Changing a roller on a conveyor. EE was holding the roller as the bolts were removed. When the last bolt was removed the roller, and co worker working above partially fell onto his shoulder and arm.

Struck by powered moving object

EE asked to borrow supervisors phone to make an urgent call regarding medical condition of daughter. His phone was dead. Supervisor asked him to step clear of the vehicle to make call. Stepped to side and back of supervisors pickup and made call. He turned his back to the truck. Supervisor began to back up striking EE knocking him to the ground. Bruised leg.

Struck against stationary object

Stepping off of last step from Cat 380H loader and rolled ankle on a rock.

EE was replacing a screen cleaning bar when a piece of cutting edge bounced onto his finger smashing and cutting it.
