Lab technician got out of his pickup to speak with operator at mine and slipped on the ice and strained right shoulder. Initially treated as first aid injury and technician returned to work without restrictions. Pain continued and work restrictions were put in place for technician restricting use of arm.

a rock fell from a moving conveyor and struck him on the mouth chipping a tooth

sliding a screen across catwalk. the screen had a sharp piece of metal from vibration of shaker and he pushed it with his foot causing a very small puncture wound to his foot
