Struck by flying object

Changing a ripper tooth on a dozer, employee was pounding the tooth into place and a small piece broke off and hit him in the chest impaling him about 1/2 inch.

Fall onto or against objects

Clearing rocks from crusher frame under jaw, he lost footing and fell into rocks and shovel.

Struck against a moving object

Employee was operating a loader that ran over a rock, he struck his head on the column in the loader.

Over-exertion in lifting objects

Lifting a tail pulley with another employee - strained hip and leg.

Climbing off of sand stacker, lost footing and slipped twisting knee.
