Accident type, without injuries

A roof fall occurred on the working section in 7 to 8 crosscut.

Hoist down at 11:30 p.m. Motor plate inside motor had broken. Employees were brought to surface. Hoist repairs were made in 1-1/2 hours. Several test runs were made after repairs. No other damage was observed.

Hoist down for repair on rope. Hoist rope caught on track fish plate. Bolts damaged 4 wires on cable. We voluntarily shut down job until repairs could be made on 05/21/2007.

Hoist down for 30 minutes to adjust brakes. All employees were brought to surface. Repairs were made, test run was made and hoist was put back in service at 1:42 A.M. on 05/03/2007.

While the slope hoist was running the falk coupling broke on the main shaft. MSHA was notified at 8:30a.m. Hoist was repaired at 3:25p.m.

Occurrence at hoist house. Hoist was not running at time of occurrence. A forklift operator for another contractor went under hoist cable with forklift. When backing up, he hooked the hoist rope with the forklift, pulled cable back causing damage to hoist house. The rope was kinked and had to be replaced with new rope. Job site down from 3:30 pm 3/24/07 until 1:30 am 3/26/07.

Struck by falling object

EE and coworker were building belt head. They were tying in belt structure. They were putting on the top rollers. EE and coworker slipped, threw the rollers and it landed on his right ankle.

Over-exertion in lifting objects

Employee was lifting a piece of 6"" pipe and as he picked it up to move it out of the walkway, he felt something pull in his left groin.

Employee bent over to pick up something and could not straighten up. Diagnosis is back spasms.

Employee hurt his back carrying conduit.

Struck by powered moving object

Employee was hit by winch cable while pulling bull hose around. He was thrown into rib. Abrasions to head, skull fracture.

Shoulder strain due to drilling. Not immediately reported.

EE was walking to his work station and stepped in a hole, causing him to slip and twist his ankle.

Struck by falling object

Chains were being removed from a fender to unload a truss from a truck. The employee was standing on the truck bed of the trailer when the load jumped up, knocking him off the trailer, causing him to sprain his ankle.

Over-exertion in lifting objects

Employee was picking up a spring for coal screen and strained muscle in his back.

Over-exertion in pulling or pushing objects

Employee pushed himself up with his right hand from a sitting position after welding, spraining his right wrist.

Over-exertion in lifting objects

Employee went to ICG job to pick up a welding machine to take to another job. In the process of picking up the tongue of the welding machine to put on the hitch, he pulled a muscle in his lower back.

Fall down stairs

Employee was painting weld pieces in steel and was going down the steps from the plant to take lunch and missed a step causing his left ankle to turn to the side.

Struck by flying object

Employee was repairing a return stopping when a piece of an Omega block went into his eye beside his safety glasses. Employee was in a high volume of air.

Struck against stationary object

Loosening bolt, wrench slipped. Hand hit a piece of decking - right hand top side of hand.

Struck by falling object

Unloading truck with forklift, one fork out of position and he pulled fork by hand to reset in position. Fork fell into position mashing right thumb.

Contact with electrical current

Employee was attempting to change batteries using jumpers when the power lead came loose from the lug and grounded against the plug housing causing burns to his right hand. He was treated and released to do a follow-up visit on 6/20/07.

Noise induced hearing loss. Employer not aware of claim of loss until February 2008.
