Accident type, without injuries

A non-injury roof fall occurred at 0530 am on our # 1 belt between survey stations 223 & 253 on our # 1 belt.

Accident type, without injuries

A roof fall occurred at the above mentioned time outby the # 3 section loading point in the # 3 entry in a neutral air course (not in a escapeway).Inby survey station 5561. The fall was approximately 40' long, 10' wide, and fell about 3' high. All approaches have had breaker timbers set and been dangered off.

While the injured was installing a fly board with a conventional chuck bolt the hole was not drilled to the proper dimension. He was forcing the bolt threw the pre-drilled hole and once it popped thru the board sprung back and caused a curtain hook to cut his palm.

As the injured was installing roof bolts a piece of draw rock fell and struck him on the foot.

As the injured was cleaning the # 3 left cross cut at or near survey station 6781 a piece of rib rock rolled out and struck him in the left shoulder.

The employee bent over to pick up and get out the way a section of miner water line. When he bent over he stated that he felt a pop in his back.

The injured and three others were going to recover some high voltage cable from old works, they had knocked an old stopping to gain access to the area, after they traveled thru the old stopping with a scoop they were attempting to hang a piece of curtain up and some block that was left on the roof had fell and struck the employee in the back of the head.

The injured was bolting in the 7 right crosscut. After he stacked his pusher and finishing piece of steel together and was drilling to his required depth, he went to get a bit out of the tray to change on his starter steel and a piece of rock fell on top of his canopy, and continued to fall on his left hand.

Caught in, under or between a moving and a stationary object

The injured was operating the bolter to install a fly board for the face of #1. Curtain was hung across the intersection to keep air in the face. After he drilled his hole and was pulling the steel down he used his hand to help bring it out and the air blew the curtain into the driller station catching the steel and wrapping his arm into the steel.

Struck by rolling or sliding object

The original date of injury was 4/26/2011, this incident was not classified a reportable incident until 5/17/2011 after a follow up visit to the physician that revealed a fractured bone in his foot and he was put off from work. The injured set the miner up in the # 7 entry and went to get the slack rope off of the machine and a rib rolled on to his foot.

Over-exertion in lifting objects

Injured stated that after lifting block over his head to build a stopping his back started to hurt and continued to get worse until the point he could not continue to work.

The injured was trying to situate the cat pads into place under the miner to change them out and strained his back in the process.

Over-exertion in pulling or pushing objects

The injured was helping to move the section tool boxes up closer to the section. When he was pulling the tool boxes up into the scoop bucket, he strained his back.

As the employee was installing roof bolts a piece of rock fell and hit his head.

Caught in, under or between a moving and a stationary object

The injured and another employee was putting a ram jack for a scoop onto a Johnson ride, as they were setting it down it slipped and caught his fingers in-between the jack and the ride.

During a belt move the injured was trying to seperate two pieces of waterline. While pushing/ pulling he cut his hand on the edge of the waterline.

The conveyor chain had broke on the continuous miner. The injured and another employee was working to get the chain back together with a chain hoist. While the witness was putting tension on the conveyor chain and the hook slipped out and struck the victim in the face.

After the injured had drilled his hole to insert a roof bolt he was putting his glue into the hole and a piece of rock fell and struck the victim on his right arm.

The injured was attempting to square off the end of some conveyor belt to make a splice, he was pulling the belt knife toward himself and the knife slipped out of the belt and cut him on the right knee.

The injured was loading roof bolt plates onto the scoop and they cut through his glove causing a laceration.

While loading a shuttle car it shoved the boom of the miner into the top and sheared a roof bolt off which came down and struck the injured on the corner of his eye.

A piece of rock struck the employee while bolting. His hand was setting on the tray of the machine when the rock fell and hit his hand.

The injured was using a belt knife to cut a piece of hose to fit for a bantam duster, while doing so, his thumb was cut by the knife.

The injured was pushing his drill steel against the mine roof to start a hole, when a piece of loose rock fell and hit on the arm.

The injured was swinging a sledge hammer to knock block out of a stopping, when he swung he hit his hand on the block.
