Accident type, without injuries

An unplanned roof fall occurred in a non-traveled intake air course outby the section loading point. The fall was approximately 60' long, 18' wide and 7-8' high. It began 40' outby survey station 6593 and continued to survey station 25631.

Struck by rolling or sliding object

Employee was dragging a piece of curtain up #9 entry to ventilate the face while they bolted it when a rib measuring 11"" thick X 29"" wide X 27"" high rolled out and struck the employee on the right ankle which resulted in a fracture to the right ankle.

Caught in, under or between a moving and a stationary object

The employee had EE's hand on the canopy post and inadvertently lowered the canopy catching EE's right middle finger between the collars.

Struck by... (Not Elsewhere Classified)

As EE was lowering the drill pot, the pot pulled off of the wrench and the wrench sprang back towards EE and struck EE's chin.
